Antimicrobial Resistance Profile of Uro-Genital Bacterial Isolates in Southern Libreville, Gabon: A Cross-Sectional Study Based on Minimum Inhibitory Concentrations (2025)  [PDF]

Abstract: Background: Antimicrobial resistance (AMR) is an escalating public health threat in sub-Saharan Africa, where local surveillance data remain limited, including in Gabon. This study aimed to describe the antimicrobial resistance profile of bacterial isolates from uro-genital infections in the southern Libreville region over the year 2025. Methods: Retrospective cross-sectional study of 477 clinical specimens processed with an automated identification and susceptibility testing system. After excluding yeasts (n = 199), 278 bacterial isolates were included. Resistance status per antibiotic class was determined from resistance phenotypes and raw minimum inhibitory concentrations (MICs); MIC50 and MIC90 values were calculated for key antibiotics in Escherichia coli, Klebsiella pneumoniae, and Staphylococcus spp. Proportions are reported with 95% confidence intervals (Wilson method). Results: The population was predominantly female (76.3%), with urine cytobacteriological examination (43.8%) and combined genito-urinary specimens (40.7%) as the leading specimen types. Escherichia coli (35.6%) and Klebsiella pneumoniae (14.7%) were the most frequent bacterial isolates. High resistance was observed for trimethoprim-sulfamethoxazole (98.3%) and beta-lactams (79.3%), with an MIC90 for cefotaxime and ceftriaxone ≥ 64 mg/L in both E. coli and K. pneumoniae, suggestive of a high prevalence of ESBL production. More than half of all bacterial isolates (56.5%) were resistant to at least three antibiotic classes, meeting a standard multidrug-resistance (MDR) definition. Direct analysis of vancomycin MICs (MIC50 0.75 - 1 mg/L, MIC90 1 mg/L) revealed no glycopeptide resistance among Staphylococcus spp., contradicting an initially discordant automated derived indicator. Conclusion: These data confirm a high level of resistance to first-line antibiotics among uropathogens in the southern Libreville region, consistent with published data from other Gabonese and sub-Saharan African sites, and highlight fosfomycin, nitrofurantoin, and carbapenems as valuable therapeutic options of last resort.Subject AreasMicrobiology

Abstract: Pancreatic cancer patients present late in their course and surgical resection as a modality of treatment is of limited value. Majority develop loco-regional failure and distant metastasis, therefore, adjuvant therapy comprising of radiotherapy and chemotherapy are useful treatment options to achieve higher loco-regional control. Specialized irradiation techniques like intra-operative radiotherapy that help to increase the total tumor dose have been used, however, controvertible survival benefit was observed. Various studies have shown improved median and overall survival with chemoradiotherapy for advanced unresectable pancreatic carcinoma. The role of new agents such as topoisomerase I inhibitors also needs further clinical investigations.
Socio-Demographics and Late Antenatal Care Seeking Behavior: A Cross Sectional Study among Pregnant Women at Kyenjojo General Hospital, Western Uganda  [PDF]

Abstract: Background: Late antenatal care attendance among pregnant women at health facilities remains a significant public health problem. Globally, approximately 830 women die every day due to pregnancy-related complications and 99% of these deaths occur in developing countries whereby 86% of pregnant women access Antenatal Care (ANC) services at least once. Objective: The general objective of the study was to determine factors associated with late antenatal care seeking behavior among pregnant women at Kyenjojo general hospital. Methods: This was a descriptive cross sectional study design that considered quantitative data collection methods among pregnant women attending ANC. The sample size was determined using Kish and Leslie (1969) formula using a proportion of 37% (0.37) of women who sought late ANC. A systematic sampling technique was used to sample pregnant women on daily basis. Results: A total of 283 women participated in the study. Spouse’s occupation was significantly associated with late ANC attendance (p = 0.026). On the other hand, education level of respondent (Fisher’s = 8.363, p = 0.028*), religion (Fisher’s = 5.77, p = 0.048*) and parity (Fisher’s 10.312, p = 0.026*) revealed statistically significant association with late ANC attendance. In multivariate logistic regression, on occupation, women with unemployed spouses were significantly associated with 25% increase in attendance of late ANC compared to those in formal employment (AOR = 0.25, CI: 0.073 - 0.855, p = 0.027*). Conclusion: The Majority of pregnant women sought ANC at 90.1% (n = 255). There’s a need for government to strengthen health promotion targeting women in rural communities.

Abstract: In recent years, the number of Secondary school Chinese language teachers has been increasing in Cameroon. Based on the “Central and West Africa” training model of the Confucius Institute in Cameroon, this study investigated the degree of satisfaction of Cameroonian secondary school Chinese?language teachers with online training during the COVID-19 pandemic through the use of questionnaire and interview methods. This study found that teachers’?satisfaction with online training ranged between?“moderately”?and?“sli-?ghtly”, with the lowest degree of satisfaction regarding the online training platform. In addition, some shortcomings with the training instructor’s per-?formance, courses content, training services and effectiveness have been identified. Furthermore, this study also revealed that the training environment, teachers’?motivation, and family burden are correlated with teachers’?satisfaction with the online training. To address these issues, this study concludes with some practical recommendations, such as the urgent need for teachers to take the initiative to actively participate in online training, in order to improve their professional knowledge, strengthen and diversify the use of online training platforms, develop a fully integrated online

Abstract: This paper examines the factors determining the adoption and use of solar photovoltaic (PVs) technologies in Uganda using detailed Uganda 2018/2019 Living Standards Measurement Survey (LSMS) household data. The data were analyzed with a Probit model and a Multivariate Probit model. We found that the major drivers of solar PVs use in Uganda are: savings, income, education, age of household head, and household size. However, households in urban areas, households with access to grid electricity, households with reliable grid electricity supply, and female-headed households are less likely to adopt solar PVs. The study recommends that the government should promote awareness of solar energy and establish credit schemes for solar provision to lessen the burden of upfront investment in solar.

Abstract: Aim: To study the epidemiological profile, clinical symptoms and referral patterns of patients with gynecological malignancy. To evaluate pain symptoms, response to treatment and factors affecting management in patients with advanced gynecological malignancies. Methods: A retrospective analysis was performed of the gynecological malignancy cases registered at the Pain and Palliative Care Clinic, Calicut, over a 12-month period between January 2006 and December 2006.Patient characteristics, symptoms and response to treatment were evaluated in detail. Results: A total of 1813 patients registered, of which 64 had gynecological malignancies. Most of the cases were referred from the Oncology Department of the Calicut Medical College. Fifty-five percent of the patients were unaware of their diagnosis. Psychosocial issues and anxiety were observed in 48%. Insomnia was seen in 52% of the cases. Pain was the most common and most distressing symptom. Adequate pain relief was achieved in only 32% of the patients. Conclusions: The number of gynecological malignancy cases attending the Pain and Palliative Care Clinic is small. Pain is the most common and distressing symptom, with only 32% of the patients achieving adequate pain relief. Poor drug compliance, incomplete assessment of pain and the lack of awareness of morphine therapy were identified as the most common causes for poor pain control.

Abstract:
Controversial climate change studies purport that predicted food insecurity and resource scarcity will intensify resource conflicts in developing nations. This belief is based on a prevalent assumption that African agricultural production systems are rigid and that their respective governments lack comprehensive adaptation ability. Therefore, I investigate whether and how effective post- drought adaptation activity is sustaining food production and livelihoods at Loitoktok district in Kenya. This study uses the theoretical three-step ecosystem service governance approach that analyzes both natural resources attributes and relational data. Results confirm a substantial decline in productivity and huge monetary losses in the agricultural sector of Loitoktok following the 2009 drought. Post-drought analysis reveals high diversification in crops and livestock that are drought-tolerant, fast maturing and high income generating such as camels, rabbits and dairy goats, horticultural and fruit production that sustain food security, income and local livelihoods. These reactive adaptation activities originate from an active public-private cooperation that promotes knowledge exchange among Loitoktok stakeholders. This cooperation is also seen in the efficient resource conflict resolution network. In conclusion, rural communities seem to be efficiently adapting to changing environmental conditions but require more financial and technical support from the government. Unfortunately, appraisal of national planned adaptation reveals effort-duplication that may divert much needed adaptation funds from being invested in research projects with multiple benefits to Kenyan food producers.
Testing the Effect of Soil Heterogeneity on Arbuscular Mycorrhiza Fungi (AMF) Contribution to Plant Productivity  [PDF]

Abstract:

Most natural soils are heterogeneous and nutrient availability and soil structure change greatly over small distances. It is still unclear whether AMF are advantageous for plants under such heterogeneous soil conditions. The objective of this study was to determine whether diverse AMF community support host plant community productivity in heterogeneous soil. It was also tested whether soil heterogeneity affects plant productivity. This was carried out in a greenhouse experiment made up of two factors: soil heterogeneity and AMF richness. Soil heterogeneity was simulated by mixing three soil types (sand, field soil and organic soil) together (homogenous soil (HM)), mixing them partly (semi homogenous (SH)) or keeping the three soil types separate in three compartments within one pot (heterogeneous (HT)). AMF richness was simulated by adding no AMF, one of four different AMF species separately, or all four different AMF together. The pots were planted with a mixture of Trifolium pratense and Lolium multiflorum. There was no effect of soil heterogeneity on total plant biomass. However, the biomass of the individual plant species was greatly affected by soil heterogeneity with Lolium being the most abundant in the heterogeneous soil and Trifolium being the most abundant in the homogenous soil. Total plant biomass did not increase with AMF richness. Moreover, opposite to the hypothesis, AMF richness was not beneficial for plant productivity in a heterogenous soil environment. However, there were significant differences in plant biomass with different AMF treatments in the SH and HT treatment indicating that effects of AMF on plant productivity are influenced by soil type. These effects on yield and AMF reflect a combination of local responses to growing conditions. The results show that AMF influence on plant yield may not always be positive but is strongly dependent on ecological elasticity and environmental condition.
