Abstract: Como propuesta de diferentes sociedades científicas se estableció el término estado fetal no tranquilizador en sustitución de sufrimiento fetal, que era considerado inespecífico. Esta revisión bibliográfica se efectuó a fin de exponer a la comunidad médica los diferentes términos con que se definen las alteraciones del bienestar fetal y la influencia que el empleo de las expresiones estado fetal no tranquilizador y riesgo de pérdida del bienestar fetal generan en la práctica de Obstetricia. Asimismo, se puso énfasis en la necesidad de buscar un lenguaje técnico más unificado y se concluyó que la formación de estos términos no determina la correspondencia existente entre la evaluación prenatal del feto y su estado al nacer. As a proposal of different scientific societies the term non reassuring fetal status was coined, substituting it by fetal distress which was considered nonspecific. This literature review was carried out in order to show to the medical community the different terms with which the changes of the fetal wellbeing are defined and the influence that the use of the expressions non reassuring fetal status and the risk of loss of the fetal well-being generate in Obstetrics. Likewise, the necessity of looking for a more unified technical language was emphasized and it was concluded that these terms do not determine the existent correspondence between the prenatal evaluation of the fetus and its status at birth.

La vida fetal: riesgos de expulsión y permanencia en útero. Cuba, 1998-2002

Abstract: El presente trabajo se propone como objetivo determinar el patrón y nivel de los componentes del embarazo desde la óptica de un proceso temporal y continuo de cambios de estados. Dichos componentes son los riesgos de expulsión y la permanencia en útero del producto de la concepción, expresados en función de la edad gestacional. También se ha incursionado en la identificación de factores de riesgo para la mortalidad fetal. La información utilizada corresponde al período 1998-2002 y proviene de los registros vitales de nacimientos y defunciones fetales, de la Oficina Nacional de Estadísticas y del Ministerio de Salud Pública respectivamente, de la República de Cuba. Con dicha información, se reconstruyeron cohortes de embarazos y fue factible el dise o de tablas de vida así como el ajuste de modelos de regresión de riesgos proporcionales tipo Cox. Los resultados apuntan a se alar en primer lugar que los riesgos de expulsión y la permanencia en útero del producto de la concepción no han sufrido cambios importantes en el período estudiado. Asimismo se constató, que las diferencias más acentuadas en estos dos aspectos se observan cuando el análisis se realiza incluyendo factores tales como el bajo peso y la experiencia previa de mortalidad fetal de la gestante, que por a adidura constituyen fuertes factores de riesgo para la mortalidad fetal.

Abstract: Existe un discurso de los medios hacia la población general, para justificar el ataque al Estado de Bienestar, y los recortes en recursos sociales de todo tipo inclusive recursos humanos: "Los Gobiernos se endeudaron demasiado, entre otras causas por el incremento del gasto social que ha llegado a ser insostenible y ahora están pagando el precio; hoy la única respuesta es la austeridad fiscal, reducir el déficit y desechar o recortar el Estado de Bienestar Social". Desde el Trabajo Social hay que darle a este discurso toda la importancia que tiene, ya que si desde esta disciplina se da por válido sin más, el Trabajo Social, se convierte en un simple instrumento al servicio de las Políticas Sociales que priman en el momento. Las Políticas Sociales en sentido amplio están conformadas por la Política Económica, Educativa, Sanitaria, y Social en sentido restringido (base para el Sistema Público de servicios sociales); es decir, todos los ámbitos que configuran el Estado de Bienestar. Por ello, especialmente en el momento actual, se requiere el refuerzo de un ámbito del Trabajo Social, el de Asesoramiento en las Políticas Sociales. Su gran repercusión se fundamenta en que: a) es un campo muy amplio que puede implementarse a nivel Estatal, Autonómico y Local; b) el tipo de Políticas Sociales aprobadas en un país, comunidad autónoma y ayuntamiento, influirán en la amplitud o restricción de la mayor parte de ámbitos restantes del Trabajo Social. There is a common discourse in the media addressed to the general public, to justify their attack against the Welfare State, the cuts in social resources of every kind, including human resources: "Governments got themselves too deeply in debt, due to, amongst other reasons, the increase of social costs which have become unsustainable. And now, they are paying the price; today, the only answer is Fiscal Austerity, a reduction of the deficit and to scrap or reduce the Social Welfare State". From Social Work, this issue must be considered with its entire importance. Because if it were accepted as valid without argument, the Social Work would become just an instrument at the service of the ruling Social Policies at any gave time. Social Policies, in the widest sense, are shaped by Economic, Education, Health and Social Policies in the narrowest sense (the basis for the Public System of social services), that is to say, every aspect of the Welfare State. Abstract: Este artículo presenta algunos avances de la investigación que venimos realizando sobre el Estado del Bienestar, la Ley de la Dependencia y su aplicación en la comunidad autónoma andaluza. En primer lugar, se contextualiza el Estado del Bienestar para, posteriormente, abordar el análisis de la Ley de la Dependencia como cuarto pilar de dicho Estado del Bienestar. Por último, se analiza la aplicación de esta Ley por parte de las diferentes comunidades autónomas espa olas, poniendo de manifiesto las diferencias que existen entre las mismas; prestando especial atención a la aplicación que se viene desarrollando en Andalucía. Para ello se utilizan los datos que proporciona el IMSERSO a noviembre de 2009. A partir del análisis de estos datos se llega a concluir el importante esfuerzo que viene realizando Andalucía en lo que a la aplicación de esta Ley respecta en comparación al resto de comunidades autónomas espa olas. This article presents some advances of a research conducted on the Welfare State, the Dependency Law, and its impementation in the autonomous community of Andalusia. We contextualise the Welfare State so as to later analyze the Dependency Law as the fourth mainstay of the Welfare State. Finally, we analyze the application of this Law in the different Spanish autonomous communities, revealing the differences among them; and with special attention to the case of Andalusia. The analysis of the data provided by the IMSERSO in November, 2009 reveals the important effort made in Andalusia to implement this Law in comparison to the rest of the Spanish autonomous communities
Los estados de bienestar ante la globalización Welfare States and Globalization

Abstract: Hoy en día, el periodo de los "treinta gloriosos" (1945-1975) aparece como la "edad de oro" de los Estados de Bienestar. A lo largo de las últimas décadas, se han beneficiado de una coyuntura muy favorable tanto a nivel demográfico, económico como a nivel cultural, y su desarrollo ha sido notable. No en vano, a partir de la mitad de los a os setenta, la situación se invierte, poniendo a prueba los sistemas de protección social. Los cuestionamientos que resultan de todo ello son, para los partidarios de una primera tesis, imputables a la competencia exacerbada vinculada a la globalización y a las políticas monetaristas llevadas a cabo que, para luchar contra el desempleo, han sustituido las políticas keynesianas, provocando la menor progresión de los salarios, la multiplicación de los empleos precarios y la reducción de las prestaciones sociales. No en vano, una segunda tesis insiste en las capacidades desiguales de adaptación a la globalización de los diferentes tipos de Estados de Bienestar. Si los países más liberales parecen adaptarse mejor al nuevo contexto económico, la crisis de 2008 ha mostrado una mayor resistencia de los modelos conservador-corporativo y socialdemócrata. The period of the "thirty glorious" (1945-1975) appears as the "golden age" of Welfare States. Throughout the last decades, the Welfare States have benefited from a very positive conjuncture both demographic, economic and cultural level; and their development has been important. However, since the middle of seventies, the situation has reversed, testing the systems of social protection. The resulting issues have developed two theses. For the supporters of the first one, the results are attributed to the big competition linked to the globalization and to the monetarist policies, which fight against the unemployment, that have replaced the Keynesian policies, provoking the minor progression of the salaries, the multiplication of the precarious employments and the reduction of social presentations. Not uselessly, a second thesis insists on the unequal capacities of adaptation to the globalization on the different types of Welfare States. If the most liberal countries seem to adapt better to the new economic context, the crisis of 2008 has showed a major resistance of the conservative-corporate and social-democratic models.

Abstract: Background: Many centers in Sudan use the reference data for fetal biometry. The recently published population-based reference either overestimated or underestimated the weight of the fetuses. Objective: To establish a national reference for fetal biometry, and weight by gestational age for singleton fetuses in Sudan. Methods: Data were collected on all singleton live births documented in the data collection sheet done at Saudi Hospital from 2015 to 2016 (n = 225). Gestational age estimation was based on the last menstrual period and fetal ultrasound thereafter. Fetal biometry and weight and other 6 fetal weight formulae were assessed. Reference data for fetal growth by gestational age were created. Results: New charts and reference equations are reported in Sudanese population for fetal biparietal diameter, head circumference, abdominal circumference and femur length and fetal weight. Conclusion: We advocate that these reference charts and equations for fetal biometry and weight might be valuable in the clinical use for appropriate ethnic Sudanese.

Abstract: Introduction: The occurrence of intrauterine fetal death (IUFD) is a traumatic event not only for the mother and her family but also for the obstetric team. IUFD is a common event. Objective: the aim of our study is to describe the epidemiological, diagnostic and management aspects of IUFD at the Sylvanus Olympio University Teaching Hospital (CHU SO), Lomé. Methodology: It was a descriptive cross-sectional study with retrospective data collection process that concerned cases of IUFD from January 1st 2023 to December 31, 2023 at CHU SO. Results: The hospital prevalence of IUFD was 2.43%. The mean age of the mothers was 30.6 ± 6 years. The Multigestures represented 32.52% and primiparous women represented 29.72%. Pregnant women were referred in 88.11% of cases; 47.55% had done at least 4 antenatal care visits and 4.20% had a history of IUFD. The etiological factors of IUFD were preeclampsia in 28.67% of cases, retroplacental hematoma in 28.67% and premature rupture of membranes in 4.55% of cases. Misoprostol was used for artificial induction of labor in 57.14% of cases and the delivery route was vaginal in 75.87% of cases. Conclusion: The frequency of IUFD is high and its reduction remains a challenge in low-income countries.

Abstract: La gestación es un estado caracterizado por un incremento, estrictamente regulado, de los procesos oxidativos en la madre y en el producto de la concepción, determinado por un aumento en el consumo de dioxígeno, y por la utilización de algunas especies reactivas en varios procesos celulares importantes para el desarrollo materno-fetal. El desbalance a corto y/o largo plazo del equilibrio antioxidantes/prooxidantes a favor de estos últimos provoca estrés oxidativo y da o a biomoléculas, pérdida de sus funciones y muerte celular, lo que pudiera afectar la evolución normal de la gestación. Por este motivo resulta importante conocer cómo se regula el estado redox en este período. En la presente revisión se muestran algunos de los principales estudios y se discuten resultados que evidencian el papel de las especies reactivas y los sistemas antioxidantes en la gestación normal. Pregnancy is a state characterizing by an increase, strictly regulated of oxidative processes in mother and in fetus, determined by an increase of dioxygen consumption, and by use of some reactive species in some cellular processes significant for mother-fetus development. Short- and long-term lack of anti-oxidant/pro-oxidant balance, favouring these later, provoke oxidative stress and damage to bio-molecules, loss of functions and apoptosis, affecting the normal course of pregnancy. Thus, it is important to know regulation of redox state in this period. In present review are showed some of main studies and results discussed evidencing role of Reactive Species and Anti-oxidant systems in normal pregnancy.
Critical evaluation of American categorization of fetal heart rate (FHR) decelerations and three tier classification—Shortcomings, contradictions, remedies and need for debate  [PDF]

Abstract:

Fetal heart rate (FHR) decelerations are the commonest aberrant feature on cardiotocograph (CTG) thus having a major influence on classification ofFHRpatterns into the three tier system. The unexplained paradox of early decelerations (head compression—an invariable phenomenon in labor) being extremely rare [1] should prompt a debate about scientific validity of current categorization. This paper demonstrates that there appear to be major fallacies in the pathophysiological hypothesis (cord compression—baroreceptor mechanism) underpinning of vast majority of (variable?) decelerations. Rapid decelerations during contractions with nadir matching peak of contractions are consistent with “pure” vagal reflex (head compression) rather than result of fetal blood pressure or oxygenation changes from cord compression. Hence, many American authors have reported that the abrupt FHR decelerations attributed to cord compression are actually due to head compression [2-6]. The paper debates if there are major fundamental fallacies in current categorization of FHR decelerations based concomitantly on rate of descent (reflecting putative aetiology?) and time relationship to contractions. Decelerations with consistently early timing (constituting majority) seem to get classed as “variable” because of rapid descent. A distorted unscientific categorization of FHR decelerations could lead to clinically unhelpful three tier classification system. Hence, the current unphysiological classification needs a fresh debate with consideration of alternative models and re-evaluation of clinical studies to test these. Open debate improves patient care and safety. The clue to benign reflex versus hypoxic nature of decelerations seems to be in the timing rather than the rate of descent. Although the likelihood of fetal hypxemia is related to depth and duration ofFHRdecelerations, the cut-offs are likely to be different for early/late/variable decelerations and it seems to be of paramount importance to get this discrimination right for useful visual or computerized system of CTG interpretation.
