Traditional Chinese Medicine as a Non-Prescription Approach to Diabetes Self-Management  [PDF]

Abstract: Type 2 diabetes mellitus (T2DM) is a prevalent chronic condition requiring lifelong self-management. We conducted a comprehensive literature review examining systematic reviews, meta-analyses, and clinical trials that evaluated Traditional Chinese Medicine (TCM) modalities—including Chinese herbal remedies, acupuncture, and mind-body exercises (Tai Chi, Qigong)—as adjuncts in T2DM care. Our analysis revealed that Chinese herbal therapies showed mixed but often positive effects on glycemic control. Notably, berberine (huanglian) lowered HbA1c and fasting glucose comparably to metformin, and Astragalus-based formulas modestly improved insulin resistance. In contrast, other herbs (bitter melon, ginseng) yielded weak or inconsistent benefits. When acupuncture was added to usual care, it produced modest but significant reductions in HbA1c and fasting glucose in pooled analyses and notably relieved diabetic neuropathic pain. Mind-body exercises (Tai Chi, Qigong) led to mild improvements in glycemic markers, with small HbA1c declines and consistently enhanced quality of life and psychological well-being. Most importantly, combining TCM modalities often provided additive effects: integrative programs (herbal + acupuncture + exercise) demonstrated superior glycemic outcomes and metabolic profiles compared to single therapies or lifestyle advice alone. However, the evidence base is limited by small, short-term trials with methodological and publication biases, so results should be interpreted cautiously. Nevertheless, this review highlights the potential of evidence-based TCM approaches as holistic, patient-centered complements to standard diabetes care. Further large-scale, rigorous trials are needed to confirm these benefits and guide safe integration of TCM into diabetes self-management.

Abstract: 中医香疗体系作为中医药体系的重要组成部分之一,在防治疾病,养生保健方面发挥着独特作用,而香薰作为中医香疗开展的重要手段之一,以其绿色天然,简便易携等优势,在当代为各阶段人群较广泛的接受。本文将在基于肯·威尔伯的整合心理学的视角下,对现代中医香疗在心理调节上的相关文献资料进行梳理总结,探讨中医香疗在当代对于情绪心理的调节作用及其机制,探索整合视域下中医香疗的“香–情–境”三维情境增强体系搭建,以期能为现代心理亚健康问题的解决提供新的思路,找到中医香疗继承发展的新方向,进一步推动中医香疗在现代生活与医疗中的应用。
As one of the important parts of Chinese medicine system, Chinese medicine aromatherapy system plays a unique role in disease prevention and health care, and aromatherapy, as one of the important means of Chinese medicine aromatherapy, is widely accepted by people of all stages of life in contemporary times due to its advantages of green and natural, simple and easy to carry, and so on. In this paper, based on Ken Wilber’s perspective of integrative psychology, we will summarise the relevant literature on the psychological regulation of modern TCM aromatherapy, discuss the role of TCM aromatherapy in the regulation of emotional psychology and its mechanism in contemporary times, and explore the three-dimensional mood enhancement system of aroma-emotion-situation of TCM aromatherapy under the perspective of integration. The three-dimensional context enhancement system of Chinese medicine aromatherapy under the integrated perspective is constructed in order to provide new ideas for the solution of modern mental sub-health problems, to find a new direction for the inheritance and development of Chinese medicine aromatherapy, and to further promote the application of Chinese medicine aromatherapy in modern life and medical treatment.
A Survey of Licensed Acupuncturists in the San Francisco Bay Area: Prevalence of Treating Oncology Patients

Abstract: Background: Many cancer patients seek traditional Chinese medicine (TCM), the prevalence varying with diagnosis, comorbidities, and demographics. Interventions sought include acupuncture, massage, herbs, diet, and exercise, usually combined with conventional therapies. It is not known what proportion of TCM practitioners care for cancer patients, their cancer specific training or caseload, what interventions they employ, their outcomes, and their communication patterns with conventional oncologists. Methods: A survey was mailed to all 2213 licensed acupuncturists in the 9-county San Francisco Bay Area gathering descriptive statistics. Results: A total of 472 (21%) responded by mail or web-based Research Electronic Data Capture (REDCap) tool. Most respondents (77%) reported caring for patients with cancer, with 29% reporting having 6 to 10 years of practice experience, and 44.2% having 0 to 20 hours of training specific to the needs of patients with cancer. Improving quality of life was reported by 94% as what their treatment offered cancer patients as well as the area where treatment was felt to have the greatest impact. The most useful TCM modalities were acupuncture (98%), herbs (79%), diet (72%), moxibustion (46%), and meditation instruction (44%). Absence of adverse reactions was noted by 95%. Ninety-one percent reported “never” or “hardly ever” having been contacted by patients’ oncologists to discuss treatment. Conclusions: Many acupuncturists seeing cancer patients have significant clinical experience and have sought specialized training. Improved communication is needed between TCM practitioners and oncologists sharing care of cancer patients

Abstract: 神经根型颈椎病(CSR)是颈椎退行性疾病中最常见的亚型之一。长期伏案、不良姿势及慢性劳损所致的颈椎力学失衡,被认为是其高发与年轻化的重要原因。神经根受压后可引发放射性疼痛、感觉异常及运动功能受限,长期疼痛还可能诱发中枢敏化和情绪障碍,进而显著降低患者生活质量。对于无进行性神经损伤者,中西医保守治疗仍是临床首选方案。西医治疗以非甾体抗炎药、神经营养药物、牵引及物理治疗为主,侧重于控制炎症、减轻水肿和改善局部受压;中医治疗则基于“筋骨失衡”、“本虚标实”的病机认识,采用中药、针灸、推拿等方法,从整体调节、力学重建及多靶点干预等层面发挥作用。文章系统梳理CSR中西医保守治疗的研究进展,重点归纳其共同作用机制,包括减轻机械压迫、下调TNF-α和IL-1β等炎症介质、改善神经根局部微循环、调节神经递质释放及促进颈椎生物力学平衡重建。同时,总结中西医结合在增效减毒、缓解症状及降低复发风险方面的潜在优势。针对当前研究中存在的疗效评价主观性较强、机制研究与临床转化衔接不足、干预参数标准化程度不高等问题,提出未来可结合多组学技术、客观生物标志物及动态力学监测手段,构建更加客观、标准化的评价体系,以为CSR保守治疗的精准化和规范化提供参考。
Cervical spondylotic radiculopathy (CSR) is a common degenerative cervical disorder characterized by radiating pain, sensory disturbance, and motor dysfunction caused by nerve root compression. With the increasing prevalence of prolonged sedentary work, poor posture, and chronic cervical strain, CSR has shown a trend toward younger onset and imposes a substantial burden on quality of life. For patients without progressive neurological deficits, conservative treatment remains the first-line approach. In Western medicine, nonsteroidal anti-inflammatory drugs, neurotrophic agents, cervical traction, and physical therapy are commonly used to reduce inflammation, relieve edema, and alleviate neural compression. In traditional Chinese medicine, herbal medicine, acupuncture, and Tuina are widely applied to regulate the whole body, restore musculoskeletal balance, and exert multi-target therapeutic effects. This review summarizes recent advances in conservative treatment for CSR from the perspectives of both Western medicine and traditional Chinese medicine, with a focus on their shared therapeutic mechanisms, including anti-inflammatory regulation, improvement of local microcirculation, modulation of neurotransmitter release, relief of mechanical compression, and restoration of cervical biomechanical balance. In addition, the potential advantages of integrated Chinese and Western medicine in symptom control, efficacy enhancement, and recurrence prevention are discussed. Future studies should further strengthen objective efficacy evaluation, standardize intervention protocols, and promote the integration of mechanistic research with clinical translation, so as to support the development of more precise and standardized conservative management strategies for CSR.
Comparative effectiveness research on patients with acute ischemic stroke using Markov decision processes

Abstract: The electronic health records (EHR) of patients with AIS hospitalized at the 2nd Affiliated Hospital of Guangzhou University of Chinese Medicine between May 2005 and July 2008 were collected. Each record was portioned into two "state-action-reward" stages divided by three time points: the first, third, and last day of hospital stay. We used the well-developed optimality technique in MDP theory with the finite horizon criterion to make the dynamic comparison of different treatment combinations.A total of 1504 records with a primary diagnosis of AIS were identified. Only states with more than 10 (including 10) patients' information were included, which gave 960 records to be enrolled in the MDP model. Optimal combinations were obtained for 30 types of patient condition.MDP theory makes it possible to dynamically compare the effectiveness of different combinations of treatments. However, the optimal interventions obtained by the MDP theory here require further validation in clinical practice. Further exploratory studies with MDP theory in other areas in which complex interventions are common would be worthwhile.Comparative effectiveness research (CER) is a way of identifying what works for which patients under which circumstances [1]. CER is not a single entity, it can take many forms, including cohort studies, literature systematic reviews, observational studies, and randomized controlled trials (RCTs) [1,2]. Non-randomized comparative clinical studies also play an important role in assessing the safety and effectiveness of medical interventions for routine practice. This article explores the application model of the integrative concept in the construction of traditional Chinese medicine (TCM) departments in primary healthcare institutions, set against the backdrop of the rural revitalization strategy. During the rural revitalization process, primary healthcare institutions are a crucial component in ensuring the health of rural residents. However, these institutions currently face issues such as resource shortages and a lack of skilled personnel, which have hindered the development of TCM departments. By analyzing the current status and problems of TCM department construction in primary healthcare institutions, this article proposes an application model for the integrative concept to promote the development and optimization of TCM departments, providing strong health support for rural revitalization.

Abstract: 本文探讨了眉山市中医医院在整合医学视野下通过中心化学科建设推动医院高质量发展的实践与成效。研究显示,医院在结构优化、创新增效、文化聚力等方面取得了不同程度的改善,特别是在中医类别执业医师比例、以中医治疗为主出院患者比例、医疗服务收入占比等方面表现突出。中心化学科建设实现了中西医协同发展的整合模式,促进了诊疗思维、服务模式和中西医关系的转变。此外,医院通过七大中心的建设实践,形成了资源整合、能力提升和效益转化的“三维驱动”发展机制,提升了医疗能级和运营效率。研究还探讨了中心化学科建设的理论创新、实践价值、管理机制和行业参考维度,分析存在的不足与未来的研究方向,包括构建综合评价模型、开展多中心研究、加强政策衔接研究和深化智慧医疗赋能等。
This article discussed the practice and effect of Traditional Chinese Medicine Hospital of Meishan in promoting the high-quality development of the hospital through the construction of centralized disciplines under the perspective of integrative medicine. The study shows that the hospital has achieved varying degrees of improvement in structure optimization, innovation and efficiency enhancement, and cultural cohesion, especially in the proportion of TCM practitioners, the proportion of discharged patients mainly treated by TCM, and the proportion of medical service revenue. The construction of centralized disciplines has realized the integrated mode of coordinated development of traditional Chinese medicine and western medicine, and promoted the transformation of diagnosis and treatment thinking, service mode and the relationship between traditional Chinese medicine and western medicine. In addition, through the construction practice of the seven centers, the hospital has formed a “three-dimensional driving” development mechanism of resource integration, capacity improvement and benefit transformation, which has improved medical energy level and operational efficiency. The study also discussed the theoretical innovation, practical value, management mechanism and industry reference dimensions of the construction of centralized disciplines, and analyzed the existing shortcomings and future research directions, such as building a comprehensive evaluation model, carrying out multi-center research, strengthening policy cohesion research, and deepening the empowerment of smart healthcare.
