Abstract: Hydronephrosis is a distention of the calyces and renal pelvis with urine as a result of obstruction of its free flow, both proximal and distal. We report a 42-year-old male patient, who attended our urological medical office presenting symptomatic urinary infection and abdominal mass. Abdominal ultrasound and CT-scan established left giant hydronephrosis, 54x30x27 cm, and loss of cortical-medullary relationship, with a 50-Lt volume, occupying all the retro peritoneum, lifting the diaphragm and compressing the bladder. Left nephrectomy was performed via laparotomy, following drainage of contents. Ureteropelvic junction stenosis was confirmed as the cause of obstruction.
