We report a 42-year-old male patient, who attended our urological medical office presenting symptomatic urinary infection and abdominal mass. Abdominal ultrasound and CT-scan established left giant hydronephrosis, 54x30x27 cm, and loss of cortical-medullary relationship, with a 50-Lt volume, occupying all the retro peritoneum, lifting the diaphragm and compressing the bladder. Left nephrectomy was performed via laparotomy, following drainage of contents.
