Histiocitosis X O De Células De Langerhans. A Propósito De Un Caso.

La histiocitosis X o de células de Langerhans es una patología que puede manifestarse con diversos cuadros clínicos de acuerdo a la extensión y el compromiso multiorgánico. Tiene poca frecuencia en ni os, adolescentes y adultos jóvenes con una incidencia de 2 por cada 200 000, con mayor afectación en varones. Su clínica es variada y de evolución impredecible. Describimos el caso de una lactante mayor femenina, de 18 meses de edad, quien ingresa con fiebre, dermatitis seborreica progresiva, hepatoesplenomegalia, linfoadenopatías y afección respiratoria compatible con histiocitosis de células de Langerhans, por lo cual se realizan paraclínicos que permiten establecer el diagnóstico y determinar el compromiso sistémico. El diagnóstico definitivo se obtuvo mediante la observación de gránulos de Birberck en el citoplasma de los histiocitos en el estudio inmunohistoquímico. Se inició tratamiento quimioterápico a base de vimblastina y prednisona, sin respuesta satisfactoria. Paciente fallece a los 45 días posteriores al diagnóstico.
