%X La histiocitosis X o de c谷lulas de Langerhans es una patolog赤a que puede manifestarse con diversos cuadros cl赤nicos de acuerdo a la extensi車n y el compromiso multiorg芍nico. Tiene poca frecuencia en ni os, adolescentes y adultos j車venes con una incidencia de 2 por cada 200 000, con mayor afectaci車n en varones. Su cl赤nica es variada y de evoluci車n impredecible. Describimos el caso de una lactante mayor femenina, de 18 meses de edad, quien ingresa con fiebre, dermatitis seborreica progresiva, hepatoesplenomegalia, linfoadenopat赤as y afecci車n respiratoria compatible con histiocitosis de c谷lulas de Langerhans, por lo cual se realizan paracl赤nicos que permiten establecer el diagn車stico y determinar el compromiso sist谷mico. El diagn車stico definitivo se obtuvo mediante la observaci車n de gr芍nulos de Birberck en el citoplasma de los histiocitos en el estudio inmunohistoqu赤mico. Se inici車 tratamiento quimioter芍pico a base de vimblastina y prednisona, sin respuesta satisfactoria. Paciente fallece a los 45 d赤as posteriores al diagn車stico. The Histiocytosis X or of Cells of Langerhans it is a pathology that can show with diverse clinical squares according to the extension and the commitment multiorganic. We describe the case of a feminine bigger lactate of 18 months of age who enters with fever, dermatitis progressive seborrhea, hepatoesplenomegaly, linfoadenopatias and compatible breathing affection with histiocytosis X or of cells of Langerhans, reason why they are carried out paracl赤nicos that allow to settle down a I diagnose and to determine the systemic commitment. The definitive diagnosis was obtained by means of the observation of granules of Birberck in the cytoplasm of the histiocytos in the study inmunohistoquimic. Treatment quimioterapic began with the help of Vimblastina and prednisona, without satisfactory answer. Patient dies to the later 45 days to diagnose.
