Abstract: the presence of a round lesion in radiography of the thorax should always lead to neoplasia being suspected; however, round pneumonia should be taken into account in differential diagnosis. it is an unusual form of pneumonia which is presented in adults, only being seen in 1% of cases, and may be associated, or not, with symptoms of acute infection and can only be accurately diagnosed by radiological and not by clinical evolution. the case of a patient having symptoms of acute infection and a round radiological lesion is presented. the patient received antibiotic treatment leading to improvement of the clinical picture, but without radiological changes leading to a pulmonary biopsy being made
