Hemangioma cavernoso intraóseo craneal Cavernous haemangioma of the skull

Abstract: Introducción. Los hemangiomas cavernosos son tumores benignos que excepcionalmente afectan los huesos craneales. En ausencia de signos radiológicos típicos, frecuentemente son intervenidos bajo la sospecha de otro tipo de neoplasia ósea, obteniéndose el diagnóstico únicamente tras el procedimiento quirúrgico. Caso clínico. Presentamos el caso de una paciente de 52 a os con una tumoración indolora del hueso frontal derecho, de lento crecimiento y características osteolíticas desde el punto de vista neurorradiológico. Para descartar un origen metastásico, se llevó a cabo un estudio oncológico sistémico, sin hallazgo de neoplasia primaria. Finalmente, la lesión fue extirpada en bloque mediante craniectomía, seguido de craneoplastia. El diagnóstico anatomo-patológico fue hemangioma cavernoso intraóseo. Conclusión. A pesar de su baja frecuencia, el diagnóstico de hemangioma cavernoso intraóseo debe ser considerado ante la presencia de una tumoración craneal de lento crecimiento y características osteolíticas en las pruebas de neuroimagen. El tratamiento de elección consiste en la resección completa de la lesión mediante craniectomía, incluyendo márgenes de seguridad de hueso sano. Introduction. Cavernous haemangiomas are benign tumours that rarely affect the skull. A correct suspicion diagnosis is seldom obtained when typical radiological signs are lacking. In this way a definite diagnosis is only obtained after a surgical procedure in most cases. Case report. A 52-year-old female presented a painless, slow-growing tumoration in her right forehead. Skull CT showed an osteolytic lesion located within the right frontal bone. On suspicion of a metastatic origin of the lesion, a sistemic research for a primary tumour was performed without significative findings. Finally, en bloc resection of the lesion was performed followed by cranioplasty. Microscopically, the lesion proved to be a cavernous haemangioma of the frontal bone. Conclusion. Despite their low frequency, cavernous haemangiomas must be included in the differential diagnosis of slow-growing osteolytic lesions located within the skull. The elective treatment of this tumours includes a complete resection by craniectomy, with safe bony margins.
Valor de la tomografía axial computarizada para el diagnóstico de tumores craneales supratentoriales Value of computerized axial tomography for the diagnosis of supratentorial brain tumors

Abstract: Se efectuó un estudio descriptivo y transversal de 94 pacientes con tumores craneales supratentoriales, hospitalizados en el Servicio de Neurocirugía del Hospital Provincial Docente Clinicoquirúrgico "Saturnino Lora Torres", desde enero del 2009 hasta febrero del 2011, a fin de mostrar el valor de la tomografía axial computarizada en el diagnóstico de estas lesiones. Entre las variables analizadas figuraron: síntomas y signos presentados al iniciarse la afección, localización primaria de la lesión, extensión a estructuras vecinas y su interpretación topográfica, así como la correlación entre el diagnóstico presuntivo por tomografía axial computarizada y el resultado anatomopatológico. En la casuística prevalecieron las féminas con edades de 45-54 a os, la cefalea como síntoma predominante, el glioblastoma multiforme como el tumor más frecuente y los hemisferios cerebrales como los más afectados, acompa ados del efecto de masa como lesión relacionada con el tumor. Asimismo, las imágenes hipodensas fueron las que más caracterizaron a los tumores supratentoriales. Además, se constató una adecuada correlación diagnóstica y anatomopatológica A descriptive and cross-sectional study was conducted in 94 patients with supratentorial brain tumors, hospitalized in the Department of Neurosurgery of "Saturnino Lora Torres" Provincial Teaching Clinical Surgical Hospital in Santiago de Cuba, from January 2009 to February 2011, in order to demonstrate the value of computerized axial tomography in the diagnosis of these lesions. Among the analyzed variables were symptoms and signs at onset of the disease, primary site of the lesion, involvement of adjacent structures and topographic interpretation, and the correlation between the presumptive diagnosis by means of computerized axial tomography and pathological findings. Females aged 45-54 years, headache as predominant symptom, glioblastoma multiforme as the most frequent tumor, and brain hemispheres as the most affected sites prevailed in the case material, accompanied by the mass effect as tumor-related injury. Also, supratentorial tumors were mostly characterized by hypodense images. Furthermore, it was found an adequate diagnostic and pathological correlation

Abstract: We studied the skulls of eleven species of Philodryas. The skull of Philodryas patagoniensis is described, and some characters that show interspecific variation are recognized. These characters are used for phylogenetic inferences. We obtained three trees using Hennig86. The pair of sister species wich is presented in the three cladograms is P. mattogrossensis - P. varius: in only ene tree: P. buroni - P. burmeisteri.
Relaciones filogenéticas de Xenodontinae (Serpentes) neotropicales basadas en musculatura craneal

Abstract: Se analizó en forma comparada la anatomía muscular craneal de 34 especies de Xenodontinae neotropicales. Se seleccionaron 22 caracteres variables con los que se realizó un análisis de filogenia del grupo, utilizándose dos especies de Natricinae como grupo externo. Se obtuvieron 139 árboles igualmente parsimoniosos que muestran las relaciones más probables entre las especies analizadas. Si bien se evidencian grupos parafiléticos, se destaca un clado bastante bien apoyado, cuyos taxones integrantes presentan características musculares particulares que los hacen altamente especializados en cuanto a la evolución de su sistema alimenticio, tanto morfológica como funcionalmente. The comparative anatomy of the cranial muscles of 34 species of neotropical Xenodontinae is analyzed. Twenty two variable characters are chosen, used in a phylogenetic analysis. Two species of Natricinae are used as outgroup. 139 trees are found that show the most probable relationships among the species analyzed. Although paraphyletic groups are evident, a prominent well supported clade is observed. The taxa of this clade present particular myological characteristics, that make their alimentary system very specialized, both morphologically and functionally.

Abstract: syndromic craniosynostosis is a difficult-to-resolve congenital anomaly generally requiring several surgical procedures. expansion of the posterior cranial vault diminishes intracranial hypertension and its deleterious effect on the brain. the use of spring-assisted distraction for posterior vault expansion showed to be an effective and stable method with lower morbidity than that seen in traditional procedures. the combination of spring-assisted posterior vault expansion and fronto-orbital advancement in a single stage before one year of life decreases the number of surgical interventions necessary and leads to adequate remodeling of the cranial vault. here we present 3 cases of syndromic craniosynostosis who simultaneously underwent both procedures with good esthetic and functional results.
Importancia del estudio antropológico forense para la calificación jurídica de una muerte violenta

Abstract: identification of a corpse in a bad condition can be complicated due to taphonomical processes and active decomposition on human remains. the combination of classical autopsy techniques, characteristic of the study of a recent corpse, and legal anthropology analysis, is fundamental in order to determine the type of lesions, the mechanism, its evolution and, as in this case, to allow a juridical qualification. in this paper we describe the case of an unidentified corpse, partially burned that appeared in the open air in a waste area. external examination showed the coexistence of carbonization, mummification, skeletal reduction and active decomposition zones. autopsy showed the existence of stitches at abdominal level as an individual trait and, as signs of violence, a multiple craneal traumatism. a skeleton has been made with the human remains and an anthropological and odontological study was carried out. starting from these analysis we obtained information about the age, sex, height and also peculiar morphological and odontological traits. these researches, in the light of the autopsy's results, have been compared with the information about missing people supplied by police. finally, the identification was possible. the pattern of the skull's fracture allowed to arrange in sequence the lesions and to consider them to the repeated action of a blunt instrument such as a hammer. the chronology of the lesions, based on the pattern of interruption of irradiated fractures, permit to assure that the first hammer's blow was made on the cráneoum's rear. this has been qualified as a treacherous act and increased the murderer's penalty requested by the public attorney.
Miología craneal de Tropidurus melanopleurus (Squamata: Tropiduridae)

Abstract: Se describe la musculatura craneal de Tropidurus melanopleurus. La misma coincide en general con la exhibida por otros Tropiduridae, para los cuales se ha postulado poseen un patrón miológico común que a la vez formaría parte de uno generalizado asignable a otros iguanios. Se destacan sin embargo como características propias para T. melanoleurus, la presencia de un mandibulohyoideus III y la ausencia de un sternothyroideus diferenciado. The cranial myology of Tropidurus melanopleurus is described. It agrees with that exhibited by other Tropiduridae, that present a common myological pattern that is included in the most generalized pattern for other iguanians. Specific characteristics that stand out are related to the presence of mandibulohyoideus III and to the absence of a differentiated sternothyroideus.
Características de la osificación craneal en Phyllomedusa boliviana (Anura: Hylidae)

Abstract: Dentro de la familia Hylidae disponemos de información sobre las secuencias de osificación craneal para menos del 4.0% de sus miembros. El presente trabajo describe la secuencia de osificación craneal de Phyllomedusa boliviana. El patrón general de osificación es similar a lo reportado para otras especies de la familia, pero existen algunas diferencias importantes tales como la osificación tardía de los próoticos y la osificación premetamórfica de neopalatino, vómer, cuadradeyugal, ptengoides y columela. El lento desarrollo ulterior de la osificación resulta en juveniles con cráneos extensamente cartilaginosos. Characteristics of the cranial ossification in Phyllomedusa boliviana (Anura: Hylidae): the ossification sequences is known for less than 4.0% of the known taxa of Hylidae. Herein, we describe the ossification sequence of Phyllomedusa boliviana. The overall pattern of ossification in this species is similar to that reported for other hylids. but it shows some important differences among which are the delayed ossification of the prootic, and the pre-metamorphic ossification of neopalatine, vomer, quadratojugal. pterygoid, and columella. Moreover, the subsequent slow development of the cranial centers of ossification results in juveniles with extensive cartilaginous skulls.

Abstract: Las craneoestenosis sindrómicas son anomalías congénitas de difícil resolución y requieren generalmente varios procedimientos quirúrgicos. La expansión posterior de la calota permite disminuir la hipertensión endocraneana y sus efectos sobre el cerebro. La utilización de resortes para la expansión posterior ha demostrado ser un método efectivo y estable, disminuyendo la morbilidad de los procedimientos tradicionales. La combinación de la expansión posterior con resortes y el avance frontoorbitario en una sola etapa, antes del a o de vida, disminuye el número de intervenciones necesarias obteniéndose una buena remodelación de la bóveda craneana. Presentamos nuestra experiencia en 3 casos de craneoestenosis sindrómica mediante la realización simultánea de ambos procedimientos con buenos resultados estéticos y funcionales. Reconstrucción craneal con implante personalizado Cranial reconstruction with a custom-made implant

Abstract: Objetivo: Este artículo describe el uso de un implante hecho a la medida para la reconstrucción de un defecto craneal. Caso clínico: Un paciente con defecto craneal producto de traumatismo craneoencefálico. Se utilizó un implante a medida del paciente de polieteretercetona (PEEK Optima-LT, Synthes). Resultados: No hubo necesidad de ajuste transquirúrgico del implante, ni se presentaron reacciones de rechazo, infección o exposición del implante en el periodo posoperatorio (3 meses de seguimiento). El paciente disminuyó la sintomatología reportada previa a la reconstrucción, y la mejoría del contorno craneal fue notoria. Conclusión: Los implantes hechos a medida se presentan como un método razonable para la reconstrucción de defectos craneales graves. Deben evaluarse diversos factores, tales como el costo, el procedimiento quirúrgico y los resultados a largo plazo. Purpose: This article describes the use of a custom-made implant for cranial reconstruction. Clinical case: In a patient with a traumatic cranial defect, a custom-made implant of Polyetheretherketone (PEEK Optima-LT, Synthes) was used for reconstruction. Results: Intraoperative implant adjustments were not required. No rejection reactions, infections or implant exposure occurred in the postoperative period (3 months follow-up). The patient's preoperative symptoms diminished and the cranial contour improved notably. Conclusion: Custom-made implants are proposed as a feasible method for the reconstruction of severe skull defects. Factors such as cost, surgical procedure, and long-term results, must be evaluated.
