Abstract: Popliteal venous aneurysms (PVAs) are very rare. More often it is an incidental finding found on duplex ultrasonography. Venous thrombosis and pulmonary emboli is the primary presentation in most cases of PVAs, hence it is a clinically significant abnormality and a potentially morbid and fatal condition. Early surgical repair should be performed immediately after the diagnosis is made because of the risk of pulmonary emboli. We report a case of symptomatic PVA presenting primarily with local symptoms and chronic pain in popliteal region.

Abstract: A popliteal artery aneurysm is defined as a dilation of the popliteal artery greater than 20 mm, or greater than 50% at diameter of the native artery. Popliteal artery aneurysms are rare, but are most common in the lower extremities. These aneurysms have a high risk of ischemic complications and amputations even in unoperated patients. We describe the case of a 54-year-old man, hypertensive, with a large popliteal artery aneurysm. Doppler ultrasound and CT angiography of the lower limbs showed a 100 × 80 mm aneurysm of the popliteal artery. This aneurysm is responsible for venous compression with edema of the tissues under the skin. The patient was treated and a flattening of the aneurysm associated with bypass by the inverted right internal saphenous vein taken from the ipsilateral leg was performed. The patient was discharged on the 6th day of the operation. The patient was seen again at the outpatient clinic for one month and in the 3rd month an arterial Doppler ultrasound of the lower limbs performed had shown patency of the venous graft and disappearance of the intermittent claudication.

Abstract: Profunda femoris vein (PFV) rarely forms a direct communication with the lower end of the femoral vein (FV) or popliteal vein (PV). During regular dissections for medical undergraduates, we came across a rare anatomical variation of PFV in the right lower limb of an 80-year-old female cadaver. PFV commenced from the PV just above its formation by the union of anterior and posterior tibial veins. It traversed the popliteal fossa on the lateral side of the popliteal artery and entered into the adductor canal after piercing the adductor magnus muscle. Finally, it emptied into the FV in the lower part of the femoral triangle. Furthermore, the PV had a small caliber than that of PFV. Deep veins of leg are the common site for formation of thrombosis. In terms of diagnosis and operative procedures, the location of thrombosis in the deep veins of lower limb is clinically of great importance. Thus detailed knowledge of the anatomical variation reported here is useful during diagnosis and treatment of deep vein thrombosis.

Abstract: popliteal artery aneurysm is relatively rare, but represents around 85% of all peripheral arterial aneurysms. it is usually presented with ischemic complication and high risk of limb loss. for that reason, its elective treatment is indicated and currently carried through with satisfactory results using endovascular techniques. we describe our experience with the use of an eptfe-covered nitinol self-expandable stent graft - fluency (bard, germany), reinforced internally with the nitinol self-expandable zilver stent (cook, usa) for the treatment of a popliteal artery aneurism.

Abstract: the true prevalence of the popliteal artery entrapment syndrome is unknown, it is estimated to be 0.16%. it is considered as the main cause of intermittent claudication in young athletes without cardiovascular risk factors. it is characterized by the extrinsic compression of the popliteal artery either caused by defects of the muscle-tendon structures or by the hypertrophy of the gastrocnemius muscle leading to the arterial entrapment and symptoms of paresthesia, intermittent claudication and coldness of the affected limb. chronic compression of the artery may lead to irreversible damage with formation of thrombi and aneurysms and the subsequent amputation of the limb. therefore, correct diagnosis and treatment are of great importance for the patient.

Abstract: Popliteal artery aneurysms lead to many uneventful conditions if left untreated. We report a case of a 63-year-old man who presented with a pain in the lower extremity as a result of a popliteal artery aneurysm. He underwent surgery through a posterior approach with saphenous vein graft interposition. The presentation, investigation and treatment of this condition is discussed.

Abstract: Background: We report in here a very unusual appearance of a duplicated popliteal vein found in a male cadaver. Popliteal vein is usually a single vein present deep in the popliteal fossa which is formed by the union of venae comitantes of the anterior and posterior tibial arteries. Methods: Anatomical variation in the lower extremity was identified during routine dissection of human cadaver. Result: We noted the presence of unilateral doublepopliteal veins which were formed at the lower end of popliteal fossa. Both the veins were found closely related to the popliteal artery and both of them received tributaries. In the adductor canal both the popliteal veins joined to form the femoral vein. Conclusion: It has been believed that such venous variations in lower limbs have important implications for diagnostic ultrasonography imaging in cases which are suspected with deep vein thrombosis. Thus this case reported in here is very helpful for surgeons for proper diagnosis and management of deep vein thrombosis and also to radiologists to aid in the interpretation of venograms. Pattern of femoro-popliteal aneurysms in an African population

Abstract: objective: to describe the pattern of femoro-popliteal aneurysms in an african kenyan population. patients and methods: records of african in-patients with diagnosis of femoral or popliteal aneurysms admitted at the kenyatta national hospital, nairobi, kenya, from january 1998 to december 2007 were examined for presentation, diagnosis, risk/comorbid factors, site, age, and gender distribution. data were analyzed using spss 13.0 and presented using tables. results: femoro-popliteal aneurysms constitute 33 out of 96 of peripheral cases (34.4%). the most common presentations were pulsatile mass (48.5%) and pain and swelling (33.3%). pain alone and bleeding occurred in 9.1% each. diagnosis was performed through doppler ultrasound (45.5%), angiography (30.3%) and ultrasonography (24.3%). aneurysms were associated with trauma (51.5%), atherosclerosis (21.2%), smoking (9.1%) and hypertension (6.1%). site distribution was common femoral (33.3%), superficial femoral (36.4%) and popliteal (30.3%). mean age was 46 years (range 13-79 years); with 20 (60.6%) of them occurring in individuals aged 50 years and younger. male:female ratio was 15:1. conclusion: in the present study, femoro-popliteal aneurysms constituted less than 40% of peripheral aneurysms, and superficial femoral artery was the most common site. they occurred predominantly in males aged 50 years and younger and were associated mainly with trauma and atherosclerosis. prevalence, site and age distribution of these aneurysms in the kenyan population differs from that described in studies of caucasian populations.
