Abstract: Background: We report in here a very unusual appearance of a duplicated popliteal vein found in a male cadaver. Popliteal vein is usually a single vein present deep in the popliteal fossa which is formed by the union of venae comitantes of the anterior and posterior tibial arteries. Methods: Anatomical variation in the lower extremity was identified during routine dissection of human cadaver. Result: We noted the presence of unilateral doublepopliteal veins which were formed at the lower end of popliteal fossa. Both the veins were found closely related to the popliteal artery and both of them received tributaries. In the adductor canal both the popliteal veins joined to form the femoral vein. Conclusion: It has been believed that such venous variations in lower limbs have important implications for diagnostic ultrasonography imaging in cases which are suspected with deep vein thrombosis. Thus this case reported in here is very helpful for surgeons for proper diagnosis and management of deep vein thrombosis and also to radiologists to aid in the interpretation of venograms. Abstract: 下肢骨折术后常并发深静脉血栓形成(deep venous thrombosis, DVT),而股骨颈骨折术后较其他下肢骨折类型更易发生下肢深静脉血栓。近几年,随着对术后并发深静脉血栓形成相关危险因素研究的深入,临床医师需要采用越来越个体化、系统化的相关危险因素作为预防指标。通过大量文献分析,本文从原发性因素、继发性因素这两个方面对致DVT的相关因素做简要综述。
Deep vein thrombosis (DVT) often occurs after lower extremity fracture, and femoral neck fracture is more likely to occur than other lower extremity fracture types. In recent years, with the deepen-ing of the research on the risk factors related to postoperative DVT, clinicians need to adopt more and more individualized and systematic risk factors as prevention indicators. Based on a large number of literature analysis, this paper briefly reviews the related factors causing DVT from two aspects: primary factors and secondary factors.

Abstract: 目的:探讨“清脉饮”联合利伐沙班对全膝关节置换术后深静脉血栓的防治效果;方法:纳入2023年7月至2024年3月在贵州中医药大学第一附属医院行全膝关节置换术,愿意配合且符合标准的60例患者作为研究对象,在护理条件、手术方式、假体厂家均一致的情况下,将其分为利伐沙班组与联合用药组,每组各30例。其中利伐沙班组仅予以利伐沙班抗凝治疗,联合用药组在利伐沙班组的基础上联合清脉饮口服治疗。比较两组患者的术后血栓发生率,对比两组术后第3天与第7天及第14天的D-二聚体、凝血酶原时间、纤维蛋白原,以及术后第15天HSS评分的优良率统计。试验结果:结合两组术后3天、术后7天、术后14天的凝血酶原时间、纤维蛋白原及D-二聚体分别比较,在同一指标、同一时间段内的差异值相比较,联合用药组与利伐沙班组的差异具有统计学意义(P < 0.05),HSS评分优良率的统计中联合用药组优良率(93.33%)明显高于利伐沙班组(80.00%)。结论:对于人工全膝关节置换术后的患者,在相同的常规护理条件下,予以利伐沙班抗凝的基础上,联合清脉饮可有效降低术后下肢深静脉血栓的发生率,同时还可减少术后疼痛等不适,在一定程度上提高了患者的生活质量。
Objective: To explore the prevention and treatment effect of “Qingmai Decoction” combined with rivaroxaban on deep venous thrombosis after total knee arthroplasty; Methods: Sixty patients who underwent total knee arthroplasty in the First Affiliated Hospital of Guizhou University of Traditional Chinese Medicine from July 2023 to March 2024, who were willing to cooperate and met the standards, were selected as the research objects. Under the condition that the nursing conditions, operation methods and prosthesis manufacturers were consistent, they were divided into rivaroxa group and combined medication group, with 30 patients in each group. The rivaroxaban group was only treated with rivaroxaban anticoagulation, and the combined medication group was treated with Qingmaiyin orally on the basis of the rivaroxaban group. The incidence of postoperative thrombosis was compared between the two groups. The D-dime, prothrombin time and fibrinogen were compared between the two groups on the 3rd, 7th and 14th day after operation, and the excellent and good rate of HSS score on the 15th day after operation was compared. Results: Combined with the prothrombin time, fibrinogen and D-dimer of the two groups at 3 days, 7 days and 14 days after operation, the difference between the combined group and the rivaroxaban group was statistically significant (P < 0.05). The excellent and good rate of HSS score in the combined group (93.33%) was significantly higher than that in the rivaroxaban group (80.00%). Conclusion: For patients after total knee arthroplasty, under the same routine nursing conditions, on the basis of rivaroxaban anticoagulation, combined with Qingmai Decoction can effectively reduce the incidence of postoperative deep venous thrombosis of lower limbs, and also reduce postoperative pain and other discomfort, to a certain extent, improve the quality of life of patients.

Abstract: Profunda femoris vein (PFV) rarely forms a direct communication with the lower end of the femoral vein (FV) or popliteal vein (PV). During regular dissections for medical undergraduates, we came across a rare anatomical variation of PFV in the right lower limb of an 80-year-old female cadaver. PFV commenced from the PV just above its formation by the union of anterior and posterior tibial veins. It traversed the popliteal fossa on the lateral side of the popliteal artery and entered into the adductor canal after piercing the adductor magnus muscle. Finally, it emptied into the FV in the lower part of the femoral triangle. Furthermore, the PV had a small caliber than that of PFV. Deep veins of leg are the common site for formation of thrombosis. In terms of diagnosis and operative procedures, the location of thrombosis in the deep veins of lower limb is clinically of great importance. Thus detailed knowledge of the anatomical variation reported here is useful during diagnosis and treatment of deep vein thrombosis.
Incidence of Post-Operative Venous Thromboembolism Using Compression Ultrasonography Following Trauma to Spine and Long Bones of Lower Extremity  [PDF]

Abstract:

Background: Venous thromboembolism is an important and potentially a fatal complication if not suspected and treated promptly. Various diagnostic modalities are available to detect Deep Vein Thrombosis (DVT), but each has its merits and demerits. Compression ultrasonography is a non-invasive method to diagnose DVT. This study was undertaken to evaluate the incidence of DVT using compression ultrasonography in the patients who underwent surgery of the spine and long bones of lower extremity following trauma. Material & Methods: All patients who underwent surgery following trauma to the spine and long bones of lower extremity were screened using Venous B-mode ultrasonography and color Doppler imaging. None of the patients received prophylaxis against DVT. The patients were selected on the basis of the following findings: 1) Immediately in patients found to have clinical DVT following surgery; 2) Every odd number of patients who underwent surgery and did not show signs and symptoms of DVT on the 7th, 14th post operative day and 6th post operative week. Results: The incidence of DVT using compression ultrasonography in the patients who underwent surgery of the spine and long bones of lower extremity following trauma was 17 percent. The incidence of DVT has no correlation with various clinical variables. Conclusion: The observed incidence of DVT is reasonably high and it is a potential risk factor for the development of pulmonary embolism.

Abstract: Background: The diagnosis of deep vein thrombosis (DVT) requires an etiological research of HIV infection. The objective was to identify the characteristics of patients with DVT of limbs without other risk factors in our context. Methods: We performed a comparative retrospective study from January 2005 to December 2012. We identified 162 cases of patients hospitalized in Medicine Department of Institute of Cardiology of Abidjan with 124 HIV-negative and 38 HIV positive patients. Results: DVT is more common in HIV positive young patients (57.8 ± 15.6 years vs 39.3 ± 10.6 years, p = 0.0001). The traditional risk factors were found in HIV negative patients. HIV positive patients had no predisposing factor for thrombosis. The ankle-femoral popliteal location (29% vs 73.7% p = 0.05) was most frequent in HIV positive patients. There was no significant difference in anticoagulant therapy: UFH (60.5% vs 52.6%; p > 0.05), LMWH (20.2% vs 7.9%; p > 0.05), AVK relay (99.2% vs 100%; p > 0.05) and general measures (elevation MI). Abstract: Deep vein thrombosis of the upper limb is a rare location of venous thromboembolic disease. Data on this form of thrombosis are limited. We report six cases collected in Dakar. The sex ratio was 0.5 and the average age was 27.6 years with extremes of 17 and 39. Painful limb edema was the most constant sign and involved the non-dominant limb in almost all patients. Thrombosis occurred in a field of peripartum cardiomyopathy in two patients and SS sickle cell disease in one patient taking oral contraception. There was neither cancer nor venous catheter in medical history. The diagnosis was made by venous Doppler ultrasound in all patients. Subclavian involvement was the most common. The thrombophilia assessment, done for two patients, was normal. It was about exertion thrombosis on three (3) patients. All the patients had received antivitamin K treatment relaying a low molecular weight heparin. Venous limb compression was associated with anticoagulation for all patients. The treatment duration was six (6) months. There were no reports of pulmonary embolism or death. One patient presented a post-thrombotic syndrome.

Abstract: Introduction: Deep vein thrombosis is a frequent disease, its origin is most often multifactorial. Venous thromboembolic disease (MVTE) and cancer are two frequently entangled pathologies. Here we report the diagnosis of deep vein thrombosis that discovered prostate cancer in an 88-year-old Guinean man. On clinical examination, there was a painful and hot swelling of the right leg, an absence of sloshing of the calf, a positive sign of Homans. The digital rectal examination revealed an enlarged prostate with an irregular surface. Cardiopulmonary auscultation was normal. The electrocardiogram showed a regular sinus rhythm at 65 cycles/min, with no sign of enlarged cavities or conduction disturbance. Venous Doppler ultrasound of the lower limbs showed the presence of an extensive acute deep venous thrombosis of the right sural vein extended to the popliteal and to the homolateral deep femoral. The reino-vesico-prostatic ultrasound concluded in a heterogeneous prostatic hypertrophy with projection of a median lobe associated with a bladder of fight with an important post voiding residue evaluated at 170 ml; the rate of specific antigen of the prostate PSA was at 84.87 ng/ml. The pathology analysis made after a prostate biopsy puncture concluded with an adenocarcinoma with a Gleason score of 3. Conclusion: The association of venous thromboembolic disease (MVTE) and neoplasia is frequent. The existence of active cancer in a patient is a known risk factor for MVTE and, conversely, the discovery of a first episode of deep vein thrombosis (DVT) or pulmonary embolism (PE) may be the mode of revelation of cancer.
