Abstract: in this study the authors used the elisa-based antigen detection tests that distinguish e. histolytica from e. dispar to examine the prevalence of e. histolytica infection in individuals from an urban slum in fortaleza, northeastern, brazil. this test has a sensitivity and specificity that is comparable to pcr and isoenzyme analysis, which is the gold standard. single stools samples were obtained from 735 individuals. the prevalence of e. histolytica infection was 14.9% (110/735) and 25.4%(187/735) for e. dispar-e. histolytica complex. the most affected age group for e. histolytica /e. histolytica-e. dispar infection was the 1-5 year olds but there was no remarkable decrease with age. there was no significant difference in colonization rates between males and females. the results from this survey demonstrate that e. histolytica is highly prevalent in the community studied. furthermore, it offers promise for the antigen detection test as a sensitive and technically simple tool for detecting e. histolytica infection in the field.

Abstract: an epidemiological and molecular study on e. histolytica and e. dispar was carried out in 428 patients with gastrointestinal symptomatology of diarrhea from different health centers in cumana, sucre state. the samples were processed through: direct examination with 0.85% physiological saline solution, temporal lugol staining, trichromic staining and the ritchie method of concentration; a sucrose gradient was used for cyst isolation. the small subunit of the 16s rna was amplified by nested, multiplex pcr for the molecular detection. the e. histolytica/e. dispar prevalences according to the direct, ritchie and trichromic staining methods were 20.09, 13.79 and 12.15%, respectively; while prevalences according to pcr for e. histolytica and e. dispar were 6.31% and 4.44%, respectively, also detecting four cases of mixed infection. sequencing of the amplified fragments of e. histolytica showed 100% homology with the sequences with strains from merida (venezuela), usa, brazil, mexico and genbank. the infections by e. histolytica and e. dispar were statistically associated with age but not with sex. the presence of mucus, blood and abdominal pain were only associated to e. histolytica infection. the moderate prevalence of e. histolytica shows the endemic status of this population and warns about the potential problem as a morbidity and mortality in sucre state. the frequency of e. dispar in this population suggests the existence of an overestimation problem in the diagnosis of amoebiasis with its clinical and epidemiological implications, and shows the poor knowledge about the true prevalences of this protozoan. the pcr allowed for the differential identification of e. histolytica and e. dispar, as well as the presence of mixed infections, making a great tool for epidemiological amoebiasis studies.

Abstract: Se realizó un estudio epidemiológico y molecular de cepas de E. histolytica y E. dispar en 428 pacientes con sintomatología gastrointestinal de diarrea, de distintos centros de salud en Cumaná, estado Sucre, Venezuela. Las muestras se procesaron a través de: examen directo con solución salina fisiológica al 0,85% y coloración temporal de lugol, coloración tricrómica y método de concentración de Ritchie, y para el asilamiento de quistes, se uso el gradiente de sacarosa. Para la detección molecular, se amplificó la subunidad peque a de ARN 16S por Nested-Multiplex PCR. La prevalencia de E. histolytica/E. dispar por el método directo fue de 20,09%, mientras que por Ritchie se obtuvo un 13,79% y con coloración tricrómico un 12,15%. Por PCR la prevalencia para E. histolytica fue de 6,31% y de E. dispar de 4,44%, detectándose cuatro casos de infecciones mixtas. La secuenciación de los fragmentos amplificados de E. histolytica indicó un 100% de homología con las secuencias de cepas de Mérida (Venezuela), Estados Unidos, Brasil, México y del GenBank. Las infecciones por E. histolytica y E. dispar estuvieron asociadas estadísticamente con la edad, pero no con el sexo. La presencia de sangre, moco y dolor abdominal resultaron asociadas sólo en los infectados con E. histolytica. La moderada prevalencia de E. histolytica indica su carácter endémico en esta población y advierte sobre el potencial problema como factor de morbilidad y mortalidad en el estado Sucre. La frecuencia de E. dispar en esta población hace presumir que existe una sobreestimación en el diagnóstico de amibiasis con sus implicaciones clínicas y epidemiológicas y demuestra el poco conocimiento sobre las prevalencias reales de este protozoario. La PCR permitió la identificación diferencial de E. histolytica y E. dispar, así como la presencia de infecciones mixtas, por lo cual es una herramienta indispensable para estudios epidemiológicos de amibiasis. Detection of Entamoeba histolytica/Entamoeba dispar in stool specimens by using enzyme-linked immunosorbent assay

Abstract: entamoeba histolytica actually comprises two genetically distinct but morphologically indistinguishable species. e. histolytica can cause invasive intestinal and extra intestinal disease, while e. dispar cannot. identification and differentiation of e. dispar and e. histolytica in stool sample by microscopy is imprecise. several weeks of culture and isoenzyme analysis are required to differentiate e. histolytica from e. dispar. in this study, we have used an enzyme-linked immunosorbent assay (elisa) for detection of e. histolytica/e.dispar and compared it with microscopy. eighty-eight samples were evaluated, trichrome staining was positive in 20.4% (18) and elisa was positive in 29.5% (26). both tests were positive in 14 (15.9%) samples, 4 (4.5%) only with direct microscopy, and 12 (13.6%) only with elisa. both tests were negative in 58 (65.9%) samples. microscopy has low sensitivity and high specificity, low negative predictive value and high positive predictive value in comparison with elisa. e. histolytica/e. dispar antigen detection elisa tests are inexpensive compared to the specific tests, yield objective results and do not require experienced microscopists and can therefore be recommended for screening of stools worldwide and the results can be taken for treatment that are fitting with its clinic.
Human Amebiasis: Breaking the Paradigm?

Abstract: For over 30 years it has been established that the Entamoeba histolytica protozoan included two biologically and genetically different species, one with a pathogenic phenotype called E. histolytica and the other with a non-pathogenic phenotype called Entamoeba dispar. Both of these amoebae species can infect humans. E. histolytica has been considered as a potential pathogen that can cause serious damage to the large intestine (colitis, dysentery) and other extraintestinal organs, mainly the liver (amebic liver abscess), whereas E. dispar is a species that interacts with humans in a commensal relationship, causing no symptoms or any tissue damage. This paradigm, however, should be reconsidered or re-evaluated. In the present work, we report the detection and genotyping of E. dispar sequences of DNA obtained from patients with amebic liver abscesses, including the genotyping of an isolate obtained from a Brazilian patient with a clinical diagnosis of intestinal amebiasis that was previously characterized as an E. dispar species. The genetic diversity and phylogenetic analysis performed by our group has shown the existence of several different genotypes of E. dispar that can be associated to, or be potentiality responsible for intestinal or liver tissue damage, similar to that observed with E. histolytica.
Entamoeba histolytica and Entamoeba dispar: differentiation by Enzyme-Linked Immunosorbet Assay (ELISA) and its clinical correlation in pediatric patients

Abstract: studies were carried out at a mexican pediatric hospital to determine the ratio between the pathogenic species entamoeba histolytica and non-pathogenic species e. dispar using an enzyme-linked immunosorbent assay (elisa) to detect the lectin (1 galactose n-acetyl d-galactosamine) of e. histolytica in feces. a close correlation was noted between the presence of the e. histolytica lectin and clinical symptoms. in the study, amebas were detected by microscopy in 120 children (either e. histolytica or e. dispar). but while almost all (13/14) of the children with e. histolytica had clinical symptoms, dysentery-feces with mocus and blood, diarrhea, cramping abdominal pain, tenesmus rectal, flatulence, vomiting and headache, almost none (1/106) of the children infected with the non-pathogenic ameba e. dispar had signs and symptoms. this suggests that much of the amebiasis diagnoses made in mexico are, in fact, due to non-pathogenic e. dispar
