Eradication Treatment of Helicobacter pylori Infection: Evaluation of Therapeutic Strategies in N’Djamena  [PDF]

Abstract: Background: Helicobacter pylori (Hp) infection is the most widespread bacterial infection in the world. The infection is generally acquired in childhood, but can persist into adulthood. Eradication therapy has undergone several modifications. The aim of this study was to evaluate the different therapeutic strategies used in the eradication of Helicobacter pylori infection in the Centre Hospitalier Universitaire La Reference Nationale of N’Djaména. Patients and Methods: This was a prospective, descriptive analytical study spread over one year, from September 2021 to September 2022. Patients at least 15 years of age presenting with dyspeptic symptoms, seen consecutively in a hepato-gastroenterology consultation and with a positive stool test for H. pylori infection, were included in the study. Equally, 1/3 of patients were treated with dual or triple therapy. The remaining third received quadritherapy. Results: A total of 268 patients were included in the study (mean age 38.40 ± 14.66 with extremes of 16 and 80 years). Males predominated in 58% of cases. Overall therapeutic efficacy was 88.9%. According to different therapeutic strategies, efficacy was 90.75% for dual therapy with PPI (Rabeprazole) and Amoxicillin. On the other hand, efficacy was 87% and 88.88% for PPI-based triple therapy and dual antibiotic therapy, and for PPI-based quadruple therapy and triple antibiotic therapy. Conclusion: H. pylori infection is a common disease in Chad. Dual therapy with rabeprazole combined with a high dose of amoxicillin over a period of at least two weeks showed similar if not better efficacy than triple or quadruple therapy.
Therapeutic Lifestyle Strategies Taught in U.S. Pharmacy Schools

Abstract: IntroductionSeveral organizations representing pharmacy and other health professions stress the importance of teaching public health topics as part of training future practitioners. The objective of our study was to assess the number of U.S. pharmacy schools that incorporate lifestyle modification topics into their curricula.MethodsWe developed an electronic survey on lifestyle modification topics and sent it to each of the 89 pharmacy schools in the United States. The survey defined lifestyle modification topics as topics that address nutrition, exercise, weight loss, smoking cessation, and alcohol use.ResultsOf 89 pharmacy schools contacted, 50 (56%) responded to the survey. Of the 50, four offer at least one required course in a lifestyle modification topic, seven offer at least one elective course, and one offers a required course that incorporates more than one lifestyle modification topic. Five required and nine elective courses were identified from the responses. Nutrition was the most commonly offered required course topic, followed by smoking cessation, exercise, weight loss, and alcohol use.ConclusionFew pharmacy schools are addressing recommendations to promote public health education through formalized didactic courses. More courses on lifestyle modification topics should be offered to pharmacy students, who will be highly accessible to the public as pharmacists and will be able to offer education to enhance public health focused on the prevention of chronic diseases.
Novel therapeutic strategies for patients with triple-negative breast cancer

Abstract: Triple-negative breast cancer (TNBC) represents a very heterogeneous group of breast diseases. Currently, the backbone of therapy for TNBC is mainly chemotherapy as there are no effective specific targeted agents approved to treat TNBC. Despite initial responses to chemotherapy, resistance frequently and rapidly develops and metastatic TNBC has a poor prognosis. Therefore, new targeted strategies are, accordingly, urgently needed. This article discusses the recent developments in targeted agents explored for TNBC, aiming to offer novel therapeutic strategies that can potentially assist in designing personalized therapeutics in the future as well as provide the basis for further research in an attempt to target TNBC

Abstract: Hyperprolactinemia is a common endocrine disorder that can be associated with significant morbidity. It can result from a number of causes, including use of medication, hypothyroidism and pituitary disorders. Depending on the cause and consequences of hyperprolactinemia, selected patients require treatment considering the underlying cause, age sex, and reproductive status. We describe a systematic review of hyperprolactinemia, including microadenomas and macroadenomas, in various clinical settings, with emphasis on newer diagnostic strategies and the role of various therapeutic options, including treatment with selective dopamine agonists. Shockingly complex: the difficult road to introducing new ideas to critical care

Abstract: In their review, Spronk and colleagues [1] address some of the very real dilemmas faced by clinicians during the resuscitation of critically ill patients with trauma or sepsis. Having made a diagnostic judgment as to the presenting problem – sepsis or trauma – the clinician must next decide on the phase of resuscitation that the patient is in – 'flow' phase versus 'ebb' phase [2]. Finally, the clinician must determine what therapeutic strategies to employ and judge when resuscitation is complete. In this commentary I take the clinician's perspective in attempting to translate what we have learned from many years of preclinical study.Additionally, I identify areas where evidence remains elusive, and therefore where the clinician's judgment, incorporating 'the art of medicine', must dominate the treatment plan.The clinician must first determine whether they are dealing simply with the resuscitation of a traumatized patient in whom, in the absence of direct cardiac injury, the prevailing pathophysiologic disease mechanism is intravascular volume loss. Alternatively, are they dealing with the more complex problem of a patient with the sepsis/systemic inflammatory response syndrome (SIRS) continuum, in which dysregulation of tissue oxygen delivery occurs because of abnormalities at all three levels of the circulation: the cardiac output, the distribution of blood flow, and blood flow distribution within organs [3]. In the former situation, resuscitation must include early diagnostic strategies that identify the cause of intravascular volume loss. For the latter, resuscitation must include strategies that identify whether the patient is infected (is it sepsis or SIRS?), and if the patient is infected then where. Debating the adequacy of and novel approaches to resuscitation in either trauma or sepsis is a moot exercise if the clinician fails to remember that source control must occur in parallel with resuscitation.Unfortunately, there remain examples in the clinical liter

Abstract: Diabetes mellitus (DM) is a group of metabolic diseases associated with hyperglycemia. The main problem is insufficient insulin production in pancreas β-cells (Type 1 DM) or insensitivity (or resistance) of the tissues to insulin (Type 2 DM). Type 2 DM (T2DM) is the most common type of DM. T2DM is typically associated with obesity, sedentary lifestyle, old age, family history of diabetes, and ethnicity. Diabetes mellitus is the most common endocrine disorder currently affecting over 170 million people worldwide. In 2030, it is predicted that 360 million people will suffer from this disease. Especially T2DM is rapidly emerging as one of the greatest global health problems of the 21st Century. As a consequence, it is expected that a rise in the complications associated with diabetes, such as ischemic heart disease, stroke, neuropathy, retinopathy, and nephropathy, will be triggered. Abstract:
Spinocerebellar ataxia type 3 (SCA3), also known as Machado-Joseph Disease (MJD), is an autosomal dominant neurodegenerative disorder that predominantly involves the cerebellar, pyramidal, extrapyramidal, motor neuron and oculomotor systems. SCA3 presents strong phenotypic heterogeneity and its causative mutation of SCA3 consists of an expansion of a CAG tract in exon 10 of the ATXN3 gene, situated at 14q32.1. The ATXN3 gene is ubiquitously expressed in neuronal and non-neuronal tissues, and also participates in cellular protein quality control pathways. Mutated ATXN3 alleles present about 45 to 87CAG repeats, which result in an expanded polyglutamine tract in ataxin-3. After mutation, the polyQ tract reaches the pathological threshold (about 50 glutamine residues); the protein is considered that it might gain a neurotoxic function through some unclear mechanisms. We reviewed the literature on the pathogenesis and therapeutic strategies of spinocerebellar ataxia type 3 patients. Conversion of the expanded protein is possible by enhancing protein refolding and degradation or preventing proteolytic cleavage and prevents the protein to reach the site of toxicity by altering its ability to translocate between the nucleus and cytoplasm. Proteasomal degradation and enhancing autophagic aggregate clearance are currently proposed remarkable therapy. In spite of extensive research, the molecular mechanisms of cellular toxicity resulting from mutant ataxin-3 remain no preventive treatment is currently available. These therapeutic strategies might be able to improve sign symptoms of SCA3 as well as slow the disease progression.

Additionally, it offers critical insights for guiding future research trajectories in this field.
