Abstract: Les acteurs de la santé au travail sont confrontés à un manque d’outils quantitatifs de suivi et de diagnostic du syndrome du canal carpien (SCC). Pour répondre à ce manque, un outil de diagnostic du SCC est proposé dans cette communication. Il est composé de deux tests : le test de la roue qui consiste à détecter des encoches sur une roue en mouvement et le test de la poutre qui détecte un niveau de sensibilité à une force exercée sur un doigt concerné par le SCC. Deux études expérimentales ont été conduites pour valider cet appareil. La première étude montre que les mesures effectuées avec l’appareil sont répétables, la seconde vise à étudier la sensibilité et la spécificité de l’outil en comparaison avec une étude électrodiagnostique. Les résultats montrent que l’appareil de détection du SCC a un excellent pouvoir discriminant permettant d’identifier les sujets sains et les sujets atteints du SCC. People involved with occupational health at work are faced with a lack of quantitative tools to monitor and diagnose Carpal Tunnel Syndrome (CTS). In order to fill this gap, we developed a device for rapid CTS diagnosis which is composed of two complementary quantitative tests: the wheel test, which consists in detecting notches on a rotating wheel, and the beam test, which consists in detecting various forces exerted on a finger. We conducted two experimental studies to validate this device. The first one demonstrated the repeatability of the measures. The second one studied the sensitivity and specificity of our tool as compared to an electro-diagnosis test. The results show that our CTS detection device has excellent discriminatory power that allows practitioners to differentiate between healthy and CTS-affected subjects. Los actores de la salud en el trabajo se enfrentan a una falta de herramientas cuantitativas de seguimiento y de diagnóstico del síndrome del túnel carpiano (STC). Como respuesta a esa limitante, se propone una herramienta de diagnóstico del STC compuesta de dos exámenes : el examen de la rueda, que consiste en detectar las muescas de una rueda en movimiento y el examen de la viga, que detecta un nivel de sensibilidad a una fuerza ejercida sobre un dedo afectado por el STC. Dos estudios experimentales fueron realizados para validar este dispositivo. El primero muestra que las medidas efectuadas con el equipo son repetibles ; el segundo busca estudiar la sensibilidad y la especificidad de la herramienta en comparación con un estudio de electrodiagnóstico. Impact socio professionnel de la libération chirurgicale du syndrome du canal carpien

Abstract: L’objectif de notre travail était d’étudier les conséquences socioprofessionnelles d’une libération chirurgicale du SCC. Il s’agit d’une étude transversale portant sur les sujets opérés pour un SCC d’origine professionnelle ; recensés dans le Service de Médecine de Travail et de Pathologies Professionnelles au CHU Tahar Sfar de Mahdia en Tunisie sur une période de 8 ans allant du 1 Janvier 2006 au mois Décembre 2013. Le recueil des données s’est basé sur une fiche d’enquête, portant sur la description des caractéristiques socioprofessionnelles, médicales, et sur le devenir professionnel des participants. Pour étudier les contraintes psychosociales au travail, nous avons adopté le questionnaire de Karasek. La durée d’arrêt de travail après libération chirurgicale du SCC était significativement liée à l’existence d’autres troubles musculo-squelettiques autre que le SCC, la déclaration du SCC en maladie professionnelle et à l’ancienneté professionnelle des salariés. Quant au devenir professionnel des salariés opérés, 50,7% ont gardé le même poste, 15,3% ont bénéficié d’un aménagement de poste et 33,8% ont bénéficié d’un changement de poste dans la même entreprise. Le devenir professionnel de ces salariés était corrélé à leurs qualifications professionnelles et au type de l’atteinte sensitive et/ou motrice du nerf médian à l’EMG. Un certain nombre de facteurs non lésionnels déterminaient la durée de l’arrêt de travail, alors que le devenir professionnel des opérés pour SCC dépendait essentiellement de leurs qualifications professionnelles et des données de l’électromyogramme. Il est certain que des travaux beaucoup plus larges permettraient d’affiner encore ces résultats

Abstract: We describe three patients with severe disabling symptoms of unilateral dehiscence of the superior semicircular canal (DSSC) who had surgical treatment. Each patient underwent a unilateral subtemporal extradural approach with resurfacing the DCCS. In all 3 patients, all symptoms were completely resolved and remained symptom free on the long term. There were no postoperative complications. Only one patient experienced a temporary CSF hypotension syndrome and some dizziness. The pseudo-conductive hearing loss improved or resolved in all patients. Surgical treatment should be considered in patients with severe, disabling DSSC symptoms. Surgical resurfacing of the DSSC is a safe and rewarding surgical technique. The long term success rate regarding the elimination of the pseudo-conductive hearing loss and resolution of vestibular symptoms outweigh the potential surgical risks of this technique in these patients.

Abstract: purpose: to compare clinical course, causes and symptoms beginning enset time in children with complete atrioventricular canal with and without down's syndrome. methods: records of 80 patients <2 years of age, were reviewed. there were 55 (69%) with down's syndrome - group i (gi) and 25 (31%) without - group ii (gii). age at synpton enset intensity, functional class, clinical repercussion and anatomic variations in patients undergoing corrective surgery were evaluated. results: mean age at symptoms onset was similar for the two groups (50 ±75 days). class ii (nyha) was more frequent in gi (31 patients - 56.5%) and class iii-iv (nyha) in gii (19 patients - 76%) p<0.005. clinical repercussion evaluation showed that congestive heart failure was present in 34 (62%) patients of gi and 21 (84%) of gii; and, pulmonary hypertension was in 21 (38%) patients of gi and 4 (16%) patients of gii p<0.04. mean pulmonary arterial pressure of 50mmhg or more was present in 68% of children with down's syndrome and in 35% of gii. clinical course until surgical correction was down hill in 33 (60%) from gi and 21 (84%) from gii p<0.03. seventy seven patients underwent surgical correction. cavc type a of the rastelli classification was predominant in both groups, gi 37 (67%) - gii 25 (100%). there or more severe valvar morphologic lesions in group ii (38%) than in group i (8%). conclusion: there seems to be a pulmonary vascular hyperreactivity predominance in down's children and cardiac insufficiency signs in the normal genetic group.

Abstract: at times in clinical neurology, the identification of a subtle clinical or radiological sign can lead to prompt diagnosis of a very rare or difficult case. we report on a patient who presented with untreatable headache and unilateral ptosis. computed tomography (ct) scan of the head did not reveal any structural cause. magnetic resonance angiogram showed absence of left internal carotid artery, which was eventually confirmed by a catheter angiography. reviewing the case, it emerged that a feature on the initial ct scan "bone window" would have confirmed the diagnosis, had it been searched for: the underdeveloped carotid canal, which is a consequence and a marker of internal carotid artery agenesis.

Abstract: guyon's canal syndrome, an ulnar nerve entrapment at the wrist, is a well-recognized entity. the most common causes that involve the ulnar nerve at the wrist are compression from a ganglion, occupational traumatic neuritis, a musculotendinous arch and disease of the ulnar artery. we describe two cases of guyon's canal syndrome and discuss the anatomy, aetiology, clinical features, anatomical classification, diagnostic criteria and treatment. it is emphasized that the knowledge of both the surgical technique and anatomy is very important for a satisfactory surgical result.
Ear abnormalities in patients with oculo-auriculo-vertebral spectrum (Goldenhar syndrome)

Abstract: oculo-auriculo-vertebral spectrum (oavs) is a rare condition characterized by the involvement of the first branchial arches. purpose: to investigate the ear abnormalities of a sample of patients with oavs. materials and methods: the sample consisted of 12 patients with oavs seen at the clinical genetics unit, ufcspa/chscpa. the study included only patients who underwent mastoid computed tomography and with normal karyotype. we performed a review of its clinical features, giving emphasis to the ear findings. results: nine patients were male, the ages ranged from 1 day to 17 years. ear abnormalities were observed in all patients and involved the external (n=12), middle (n=10) and inner ear (n=3). microtia was the most frequent finding (n=12). the most common abnormalities of the middle ear were: opacification (n=2), displacement (n=2) and malformation of the ossicular chain. agenesis of the internal auditory canal (n=2) was the most frequent alteration of the inner ear. conclusions: ear abnormalities are variable in patients with oavs and often there is no correlation between findings in the external, middle and inner ear. the evaluation of these structures is important in the management of individuals with oavs
