Transition of pemphigus vulgaris to pemphigus foliaceus

Abstract: We report a case of pemphigus vulgaris in a 46-year-old woman who presented with scaly and crusted erosions over erythematous base associated with itching and burning mainly over the chest, submammary folds, face, abdomen and the back. Histopathology was consistent with pemphigus foliaceus. The patient responded to oral prednisolone (60 mg/day). The above findings suggest that pemphigus vulgaris showed a transition to the pemphigus foliaceus variant.

Abstract: forty one cases of pemphigus vulgaris and thirty cases of pemphigus foliaceus were investigated at hospital universitário clementino fraga filho from 1978 to 1999. they were divided into two treatment groups: one group received up to 100 mg of oral prednisone daily and the other group received >120 mg daily. the dose up to 100 mg provided good initial control of pemphigus vulgaris and pemphigus foliaceus and did not increase the mortality rate associated to disease. the dose >120 mg induced higher morbidity. these data allowed us to establish a regimen of oral prednisone (1-2 mg/kg/daily) with maximum of 120 mg daily in the treatment of pemphigus vulgaris and pemphigus foliaceus.

A clinico-pathological study of 70 cases of pemphigus

Abstract: A clinicopalhological study of 70 cases of pemphigus observed over a span of four and a half years from January 1992 to June 1996 at the Sir J.J. Group of Hospitals and Grant Medical College, Mumbai is reported. Pemphigus vulgaris constituted the single largest group of 43 cases, followed by pemphigus foliaceus (25 cases) and pemphigus vegetans (2 cases). Majority of the cases were seen in the age group of 21-60 years, with a slight male predominance. The youngest patient was 14 years while the eldest was aged 75 years. Mucosal involvement was seen in 31 cases of pemphigus vulgaris, as opposed to only 5 cases of pemphigus foliaceus. Flaccid bullae were present in 100% cases. Pruritus was complained of in 14 cases, though it was more common in pemphigus vegetans and vulgaris. Salient histopathological features of pemphigus vulgaris observed were (I) intraepidermal suprabasal blisters (35 cases), (2) presence of acantholytic cells (40 cases), (3) "Row of tombstone appearance" (I8 cases) and (4) acantholysis involving follicular sheath (20 cases). Main histopathological features of pemphigus foliaceus were (1) subcorneal blister (15 case), (2) acantholysis (24 cases) and (3) bulla cavity containing inflammatory infiltrate (12 cases). Both cases of pemphigus vegetans showed hyperkeratosis, papillomatosis and irregular acanthosis with intra-epidermal eosinophilic abscesses besides suprabasal lacunae.

Abstract: introduction: anal involvement in patients with pemphigus vulgaris has rarely been reported. we report a case of a 76-year-old woman with muco-cutaneous pemphigus vulgaris on systemic corticotherapy who started with hematochezia and anal pain associated with constipation and difficult defecation. flexible sigmoidoscopy was normal but proctologic examination revealed skin tags with multiple erosions. after local corticotherapy and normalization of bowel movements, healing of anal lesions was seen.

Abstract: pemphigus vulgaris is a chronic and serious auto-imune vesiculo-bullous disease that affects skin and mucous membrane. antidesmosomal antibodies dissolve intercellular substance that keeps epithelial cells together, resulting in an intraepithelial blister just above basal epithelial layer. without treatment, pemphigus vulgaris leads patient to die. the fact that this disorder presents oral manifestation in most cases, in addiction to the relevance of a precocious diagnosis to the success of the treatment and prognosis support the importance of about the knowledge of this disease to dentist's notice.

Abstract: clinical case: a patient with a conjunctival blister was diagnosed with pemphigus vulgaris by immunofluorescence tests performed on a conjunctival biopsy. discussion: pemphigus vulgaris is an uncommon but serious autoimmune disease that produces blisters of the skin and mucous membranes. ocular findings are rare, but include conjunctivitis and marginal eyelid erosions. conjunctival blisters and erosions related to this condition have not been previously reported in the literature. this diagnosis can be made through direct immunofluorescence tests performed on biopsy samples of affected tissue. unless the condition is properly diagnosed and treated, it has a high mortality.
Prognostic factors of pemphigus vulgaris disease: a study on 119 patients

Abstract: "nBackground: Since the systemic steroids are introduced in Pemphigus Vulgaris treatment, the prognosis of disease improved significantly. This study was designed to evaluate determining factors in the prognosis of pemphigus vulgaris in Iranian patients. "nMethods: In this study, 119 patients with documented pemphigus vulgaris who had presented to Razi Hospital from 2001 until 2003 were included. These patients had presented for the first time and treated with prednisolone and Azathioprine. Morality rate, minor and major relapses and duration of first remission had been defined as prognostic criteria and correlation between them and other demographic variables and disease characteristics were investigated. "nResults: The majority of patients (84.1%) were followed for more than one year. The major recurrence and minor recurrence occurred in 28(23.5%) and 65(54.6%) of patients respectively, no case of mortality was observed. In patients who received treatment six months or less after onset of disease the frequency of major recurrence was less than the others. 18(17.8%) vs. 12(41.4%), (p=0.009). Duration of primary remission more than one year was detected in most of the patients (64.7%). In patients with less than 10 initial cutaneous lesions, period of primary remission was longer than the other patients. (p=0.009). Shorter duration of primary remission were noted in older patients (age>50) in comparison with younger patients (age≤50, p=0.04). "nConclusions: Male gender, old age, interval more than 6 months between onsets of symptoms to initial treatment and more than 10 skin lesions on admission, are associated with poor prognosis of pemphigus vulgaris.
Mycophenolate mofetil as adjuvant in pemphigus vulgaris

Abstract: Pemphigus vulgaris (PV) is a life threatening autoimmune blistering disease of skin and mucous membranes. Advent of systemic steroids has greatly reduced the mortality rate. However, steroids and adjuvant immunosuppressive therapy are nowadays frequent contributory agents of morbidity and mortality of PV. Mycophenolate mofetil (MMF) has been reported to be an effective adjuvant to systemic steroids. It helps in increasing the immunosuppressive effect and minimizing the toxicities by steroid sparing effect. However, its efficacy in refractory cases of PV is not well documented. The lowest possible dose with satisfactory therapeutic efficacy and least side effects is known. We used MMF 1 g/day and systemic steroids in 3 Indian patients with pemphigus vulgaris who were resistant to systemic steroid monotherapy or combination treatment with azathioprine. In our experience, MMF offers an effective adjuvant with minimal side-effects in the treatment of resistant PV.
