Laparoscopic myomectomy of a subserous pedunculated fibroid at 14 weeks of pregnancy: a case report

Abstract: We report the case of a 31-year-old Caucasian woman with acute recurrent abdominal pain due to a subserous fundic myoma, measuring 48 × 52 × 63 mm, with an implantation base of 22 × 18 mm, which was successfully treated by laparoscopy at 14 weeks of pregnancy. At a gestational age of week 40, the patient spontaneously delivered a healthy 3216 g girl baby.As far as we know, this is the first reported case of laparoscopic myomectomy this early during a pregnancy. Our experience together with an analysis of cases reported in the literature suggests that myomectomy during pregnancy may be considered safe, but only in the hands of experienced laparoscopic surgeons. There are a few reports in the literature about laparoscopic myomectomy during the first half of pregnancy that demonstrate its feasibility in selected cases. Some technical tools could improve the procedure with a minimum of risk for the ongoing pregnancy.Uterine leiomyomas are seen in 1.6% to 4% of pregnancies. With the increasing age of obstetric patients, more cases are being encountered during pregnancy. The effect of leiomyomas on pregnancy depends on their number, size and location. Myomectomy is generally avoided during pregnancy because increased vascularity can lead to hemorrhagic complications that may necessitate hysterectomy, but it is indicated in some situations [1-3]. Indications for myomectomy during pregnancy include severe abdominal pain due to torsion of subserous pedunculated myomas or red degeneration not responding to medical treatment, and an increase in myoma size causing abdominal discomfort [4,5].There are a few reports in the literature about laparoscopic myomectomy during the first half of pregnancy that demonstrate its feasibility in selected cases [1,6-10]. Adding Agnus Castus and Magnolia to Soy Isoflavones Relieves Sleep Disturbances Besides Postmenopausal Vasomotor Symptoms-Long Term Safety and Effectiveness

Abstract: The effectiveness for vasomotor symptoms and sleep disorders plus the long-term safety of a nutraceutical combination of agnus-castus and magnolia extracts combined with soy isoflavones (SI) and lactobacilli were assessed in postmenopausal women. A controlled study was carried out in menopausal women comparing this nutraceutical combination (ESP group) with a formulation containing isoflavones alone (C group) at the dosage recommended. The Kuppermann index, The Pittsburgh Sleep Quality Index (PSQI), and Short Form 36 (SF-36) were determined at baseline, three, six and 12 months. Endometrial thickness, mammary density and liver function were evaluated at baseline and after 12 months. One hundred and eighty women were enrolled in the study (100 in the ESP group and 80 in the C group). At the end of the treatment, mammary density, endometrial thickness, and hepatic function did not show substantial differences between groups. The Kuppermann index and particularly the tendency for hot flashes progressively and significantly decreased in frequency and severity during ESP versus C treatment. At the same time, a significant increase in sleep quality and psychophysical wellness parameters was observed in the ESP versus C groups. No adverse events were observed. Agnus-castus and magnolia, combined with SI + lactobacilli, can effectively and safely be used in symptomatic postmenopausal women, mainly when quality of sleep is the most disturbing complaint. The endometrium, mammary glands and liver function were unaffected after 12 months of treatment
An Integrated Approach Based on Multiplexed Protein Array and iTRAQ Labeling for In-Depth Identification of Pathways Associated to IVF Outcome

Abstract: The emergence of high-throughput protein quantification methodologies has enabled the comprehensive characterization by longitudinal and cross-sectional studies of biological fluids under physiological and pathological conditions. In particular, the simultaneous investigation of cytokines and growth factors signaling pathways and their associated downstream effectors by integrated multiplexed approaches offers a powerful strategy to gain insights into biological networks and processes in living systems. A growing body of research indicates that bioactive molecules of human reproductive fluids, including human follicular fluid (hFF), may affect oocyte quality, fertilization and embryo development, thus potentially influencing the physiopathology of pregnancy-related conditions. In this work, an iTRAQ labeling strategy has been complemented with a multiplexed protein array approach to analyze hFFs with the aim to investigate biological processes and pathways related to in vitro fertilization (IVF) outcome. The iTRAQ labeling strategy lead to the quantification of 89 proteins, 30 of which were differentially expressed in hFFs with successful compared to unsuccessful IVF outcome. The targeted study, based on multiplexed antibody protein arrays, allowed the simultaneous quantification of 27 low abundance proteins, including growth factors, chemokines and cytokines endowed with pro- and anti-inflammatory activity. A significant number of differentially regulated proteins were involved in biological functions related to blood coagulation, acute phase response signaling and complement system. Overall, the present results provide an integrated overview of protein changes in hFFs associated to IVF outcome, thus improving current knowledge in reproductive medicine and fertility research.

Abstract: In this brief consideration of mine, trying to remain as faithful as possible to the texts taken into exam, I will first try to introduce the concept of history as contemporary history in the thinking of Benedetto Croce, whereas the second part of the reasoning will point toward proving and meditating on how such concept crosses the whole philosophical system of the Italian intellectual. What does contemporary history mean according to Benedetto Croce? Why is the spirit of the historian fundamental in the description of this concept? What kind of connection exists between story and contemporary history? What does pathology of history mean? How does the concept of contemporary history relate to that of awareness, of finished and thinking? These are but a few of the questions to which I tried to find an answer with this essay.

Abstract: Despite the research on trauma, which until recent times has been directed towards the category of adults, today scholars need to turn their attention to children and adolescents who may undergo traumatic experiences at different stages of their growth. There is a prevailing hypothesis that the narrow and partial cognitive processes of memory, attention and dysfunctional problem solving skills constitute the main problem of exposure to trauma, which also compromises the family equilibrium, taking into account that traumatic events are arbitrary and unpredictable. It is possible that dissociation phenomena and derealization emerge in response to a traumatic event. The specificity and complexity of the treatment of disorders resulting from traumatic development have prompted specialists of different orientations to formulate guidelines and treatment protocols to guide clinical work.

Abstract: The terms that describe a non-consensual sexual behavior such as rape, sexual abuse or sexual violence are very different, as are the negative effects on a victim of a physical and psychic nature, taking into account the different approaches of the attacker. The consent of the victim is absent or obtained by the attacker with physical and/or psychological violence; but it is also possible that the victim is unconscious or incapable of understanding. Those guilty of sexual offenses fall into a special category that is called “sex offenders” within which there is a diversification of ways concerning the manner, degree and motivations. Research has shown that attackers acting in a group and those who are an individual attacker differ in their attitude towards the victim.

Abstract: The purpose of this paper was the analysis of the relationship between boredom and deviance behavior; this is a field not completely explored by the sociology of deviance, despite the increase in alarm due to the increase in: deviant behavior by young people and crimes committed by groups. After a brief presentation of the most important contribution in literature, to deepen this issue, we use data collected from a survey using as principal instrument, the Boredom Proneness Scale, BPS, (Farmer & Sundberg, 1986) to identify the possible forms of prevention to be adopted in family, social and institutional context, in order to not compromise, immediately or in the long term, social, physical and psychological young person well-being. To test the evidence, it was decided to implement a model, in order to describe and analyze empirical phenomena, introducing a statistical relationship, since it is not possible to know an exact relationship between variables. The model results allow to observe a direct relationship between the BPS and variables analyzed.
