从《黄帝内经》论治糖尿病周围神经病变
Treatment of Diabetic Peripheral Neuropathy from “Huangdi Neijing”

糖尿病周围神经病变是糖尿病常见的慢性并发症之一，早期主要表现为肢体远端对称性感觉、运动神经障碍，据此临床表现当以“痹证”论治。《黄帝内经》作为中医理论的奠基之作，对糖尿病及其并发症的认识有着众多独特的见解。通过对《黄帝内经》中关于“痹证”论述的整理，分析得出风、寒、湿邪等外邪侵袭为本病的主要发病因素，主要病机则为气阴亏耗、经脉痹阻、肌肤失养。笔者在跟师学习与临床实践中常会遇到此类患者，临床选取合谷刺针刺手法结合烧山火行针手法，针刺阿是穴、足三里、三阴交进行治疗，针感以热为度。
Diabetic peripheral neuropathy is one of the common chronic complications of diabetes. In the early stage, it mainly manifests as symmetrical sensory and motor nerve disorders in the distal limbs. According to these clinical manifestations, it should be treated as “arthralgia syndrome”. As the foundational work of traditional Chinese medicine theory, the “Huangdi Neijing” offers numerous unique insights into diabetes and its complications. By collating the discussions on “arthralgia syndrome” in the “Huangdi Neijing”, it is analyzed that the main pathogenic factors of this disease are the invasion of external pathogens such as wind, cold and dampness, and the main pathogenesis is the deficiency of qi and yin, obstruction of meridians and malnutrition of the skin. In the process of learning from my teacher and clinical practice, I often encounter such patients. In clinical treatment, I usually apply the “Hegu-needling” technique combined with the “Shaoshanhuo” needling technique, and needle the Ashi points, Zusanli points and Sanyinjiao points. The needle sensation should be warm to the extent.
