Abstract:

目的:观察富血小板血浆注射对肩袖部分全层撕裂的治疗疗效。方法:选取2023年1月至2024年5月在青岛大学附属医院就诊的肩袖部分全层撕裂患者27名,在超声引导下向撕裂部位及周边注射PRP 5 ml。于治疗前、治疗后4周、8周、12周记录患者的视觉模拟评分(VAS)、肩关节Constant-Murley评分(CMS);在随访期间观察并记录患者的不良反应。结果:治疗后4周、8周、12周的VAS评分均较治疗前显著降低(P < 0.05),且治疗后8周、12周的VAS评分均较前一时间点显著降低(P < 0.05);治疗后4周、8周、12周的CMS评分均较治疗前显著升高(P < 0.05),且治疗后8周、12周的CMS评分均较前一时间点显著升高(P < 0.05);除注射后疼痛外,未出现其他不良反应。结论:富血小板血浆注射治疗肩袖部分全层撕裂可缓解患者的肩关节疼痛,改善功能障碍,疗效显著,且不良反应小,具有临床应用价值。
Objective: To observe the efficacy of platelet-rich plasma (PRP) injection in the treatment of focal full-thickness rotator cuff tear. Methods: 27 patients with focal full-thickness rotator cuff tear who were treated in the Affiliated Hospital of Qingdao University from January 2023 to May 2024 were selected. Under the guidance of ultrasound, 5 ml of PRP was injected into the tear site and its surrounding areas. The Visual Analogue Scale (VAS), Constant-Murley Score (CMS) and adverse reactions of the patients were recorded before treatment, and 4 weeks, 8 weeks and 12 weeks after treatment. Results: The VAS scores at 4 weeks, 8 weeks and 12 weeks after treatment were all significantly lower than those before treatment (P < 0.05), and the VAS scores at 8 weeks and 12 weeks after treatment were also significantly lower than those at the previous time points (P < 0.05). The CMS scores at 4 weeks, 8 weeks and 12 weeks after treatment were all significantly higher than those before treatment (P < 0.05), and the CMS scores at 8 weeks and 12 weeks after treatment were also significantly higher than those at the previous time points (P < 0.05). Except for pain after injection, no other adverse reactions occurred. Conclusion: Platelet-rich plasma injection in the treatment of focal full-thickness rotator cuff tear can relieve the shoulder joint pain of patients, improve dysfunction, and has few adverse reactions, thus possessing clinical application value.
