超声引导下腹横肌平面阻滞(TAP)与腹横筋膜平面阻滞(TFP)临床应用现状分析

Abstract:
加速康复外科理念(Enhanced Recovery after Surgery, ERAS)是一项逐渐兴起的旨在通过循证指南来优化术前、术中和术后护理质量的举措。术后多模式镇痛是ERAS的一个重要环节,腹壁阻滞就是多模式镇痛的重要组成部分,可以使镇痛效果提升,同时减少使用的阿片类药物剂量,减轻因使用阿片类药物导致的不良反应,这篇文章主要探讨超声引导下腹横肌平面阻滞(TAP)与腹横筋膜平面阻滞(TFP)这两种腹壁阻滞方式的概念,解剖位置以及现阶段临床工作中的用途,并在不同手术中将两者分别与其它镇痛方式进行比较,观察这两种腹壁阻滞方式的优缺点,又将两者之间进行比较,从而能够全面认识这两种腹壁阻滞方式的优点和不足,从而帮助我们找到更好的腹壁阻滞方式,完善多模式镇痛效果。
Enhanced recovery after surgery (ERAS) is an emerging initiative to optimize the quality of care before, during, and after surgery through evidence-based guidelines. Postoperative multimodal analgesia is an important part of ERAS, and abdominal wall block is an important component of multimodal analgesia, which can improve the analgesic effect, reduce the dosage of opioids, and alleviate the adverse reactions caused by the use of opioids. In this paper, we mainly discuss the concept, anatomical location and use of ultrasound-guided transversus abdominis plane block (TAP) and transversalis fascia plane block (TFP) in clinical work at this stage, and compare the two methods with other analgesic methods in different operations to observe the advantages and disadvantages of these two methods. By comparing the two methods, we can fully understand the advantages and disadvantages of these two methods of abdominal wall block, so as to help us find a better method of abdominal wall block and improve the effect of multimodal analgesia.
