Abstract:

膈肌是最重要的呼吸肌,而膈神经是支配膈肌的唯一运动神经,各种因素致膈神经受损均可导致膈肌功能障碍,进而导致呼吸功能障碍,临床常规检测方法,如影像学、超声、肺功能等均难以准确诊断膈神经受损程度及性质。膈神经传导(Phrenic nerve conduction)及膈肌复合动作电位(Compound motor action potential, CMAP)的测定是评价膈神经损害后致呼吸功能障碍的客观指标之一。本文对经皮电刺激膈神经传导及膈肌复合肌肉动作电位测定的检测方法、评价指标与临床意义、临床应用等研究进行综述。
The diaphragm is the most important respiratory muscle, and the phrenic nerve is the only motor nerve innervating the diaphragm. Various factors can lead to phrenic dysfunction, which in turn leads to respiratory disfunction. Routine clinical testing methods, such as imaging, ultrasound, and pulmonary function, make it difficult to accurately diagnose the degree and nature of phrenic nerve damage. The determination of phrenic nerve conduction and compound motor action potential (CMAP) is one of the objective indicators to evaluate respiratory dysfunction after phrenic nerve damage. This article reviews the detection methods, evaluation indexes, clinical significance, and clinical application of transcutaneous electrical stimulation of phrenic nerve conduction and diaphragmatic complex muscle action potential determination.
