我院静脉用药配置中心管理外购药品实践与探讨
Practice and Discussion on Management of Purchased Drugs in the Pharmacy Intravenous Admixture Service of Our Hospital

目的：探讨管理外购药品的方法与效果。方法通过统计2022年1~12月我院静脉用药配置中心(Pharmacy Intravenous Admixture Service, PIVAS)的外购药品使用情况。收集外购药品在PIVAS中全流程上发现的问题，对数据和问题进行分析与讨论。结果：我院PIVAS 2022年1~12月配置的外购药品医嘱共3672条，药品品种41个。通过整理外购处方、PIVAS每日例会记录与不合理医嘱记录，发现的问题有，医师在院内HIS系统开具医嘱与在社会药店电子处方平台开具的处方上的药品数量不符、药师对外购药品基本信息不了解、外购药品的使用方法与工作流程有冲突。结论：外购药品起着对医院药品目录查缺补漏的作用，在治疗罕见病，危重症患者时起着不可或缺的作用。完善外购药品在静脉药物配置中心的管理，可以保证患者的用药合理性与用药安全。
Objective: To explore the method and effect of management of purchased drugs. Methods: Intravenous Admixture Service (PIVAS) of our hospital was used to collect problems in the whole process of purchased drugs in PIVAS through statistical analysis of their use from January to December 2022, and the data and problems were analyzed and discussed. Results: A total of 3672 orders for purchased drugs were allocated in PIVAS of our hospital from January to December 2022, including 41 drug varieties. Through sorting out the PIVAS daily meeting records and unreasonable orders records of outsourced prescriptions, we found that the quantity of drugs prescribed by the doctor in HIS system in the hospital is inconsistent with that prescribed on the electronic prescription plat-form of social pharmacies, the pharmacist did not understand the basic information of the drugs purchased from abroad and the use of the purchased drug is in conflict with the workflow. Conclusion: The purchased drugs play an important role in checking the hospital’s drug catalog, and play an indispensable role in the treatment of rare diseases and critically ill patients. Improving the management of the purchased drugs in the intravenous drug distribution center can ensure the rationality and safety of drug use for patients.
