超声引导下微波消融术治疗甲状旁腺腺瘤相关原发性甲状旁腺功能亢进症的有效性及安全性分析

Abstract:
目的:探讨超声引导下微波消融术(microwave ablation, MWA)治疗甲状旁腺腺瘤(parathyroid ad-enoma)所致原发性甲状旁腺功能亢进症(primary hyperparathyroidism, PHPT)的安全性、有效性及优势性。方法:选取2022年就诊于青岛大学附属医院并确诊为原发性甲状旁腺功能亢进症,超声发现具有甲状旁腺腺瘤并进行MWA的患者26例。观察MWA后患者临床症状的改善程度,血钙、血磷、血清全段甲状旁腺激素(intact parathyroid hormone, iPTH)的变化水平,同时在随访期间关注术后并发症的发生情况。结果:中位随访时间为6.38月,MWA后,所有患者临床症状均好转,至末次随访,血钙、iPTH明显下降,血磷持续处于正常水平,1例患者出现术后疼痛,镇痛治疗后次日缓解,无1例患者出现术后声音嘶哑,低钙血症等不良反应。结论:超声引导下微波消融术治疗甲状旁腺腺瘤所致原发性甲状旁腺功能亢进症具有良好的安全性及有效性。
Objective: To investigate the safety, efficacy and superiority of ultrasound-guided microwave abla-tion (MWA) in treating primary hyperparathyroidism (PHPT) caused by parathyroid adenoma. Methods: A total of 26 patients diagnosed with PHPT with parathyroid adenoma by ultrasound were selected from the Affiliated Hospital of Qingdao University in 2022. The improvement of clinical symptoms, intact parathyroid hormone (iPTH) and serum calcium, phosphorus were observed be-fore and after MWA. Besides, the occurrence of postoperative complications was noted during fol-low-up. Results: The median follow-up time was 6.38 months. The clinical symptoms of all patients were improved after MWA. Until the last follow-up, the blood calcium and iPTH decreased signifi-cantly, and the blood phosphorus remained at the normal level. Postoperative pain was observed in one patient, which was relieved the next day after analgesic treatment, while no hoarseness, hy-pocalcemia and other adverse reactions were observed in patients. Conclusion: Ultrasound-guided microwave ablation is safe and effective in the treatment of primary hyperparathyroidism caused by parathyroid adenoma.
