ALT正常的慢性HBV感染者抗病毒治疗必要性研究进展

Abstract:
对ALT (Alanine transaminase)正常的慢性HBV感染者群体，针对这些患者何时进行抗病毒治疗一直是存在争议与讨论。调查证实，ALT长期正常的慢性HBV感染者(People with chronic HBV infection)的肝脏细胞也可以出现严重的炎症反应或纤维化，部分可隐匿发展，治疗有效性与可负担度的提高为治愈铺垫了通道，及时进行抗病毒治疗，有助于减缓病情发展。非侵入性检测以及必要时行肝穿刺可用作诊断工具，对存在肝脏炎症/纤维化的感染者进行积极抗病毒治疗。本文主要就ALT正常的慢性乙型肝炎的研究现状、肝组织病理改变情况、抗病毒治疗疗效，以及健康管理为切入点，来论述了抗病毒治疗的必要性。
For chronic HBV-infected people with normal ALT (Alanine transaminase), it has been controversial and discussed when to treat these patients with antiviral drugs. Investigations have confirmed that liver cells of chronic HBV infection with long-term normal ALT can also have severe inflammatory reactions or fibrosis, some of which can develop insidentially, and the improvement of treatment effectiveness and affordability paves the way for cure, and timely antiviral therapy helps to slow down the development of the disease. Noninvasive testing and, if necessary, liver aspiration can be used as diagnostic tools to initiate aggressive antiviral therapy in infected patients with liver in-flammation/fibrosis. This article mainly discusses the research status of ALT on normal chronic hepatitis B, the pathological changes of liver tissue, the efficacy of antiviral therapy, and the need for antiviral therapy from the starting point of health management.
