85例甲状腺功能减低儿童生长发育状况分析

Abstract:
目的:探讨甲状腺功能减低儿童生长发育状况。方法:选取2014年6月~2017年6月在济宁医学院附属医院经新生儿筛查确诊为甲状腺功能减低患儿85例设为病例组(原发性甲状腺功能减退(CH)组53例、高促甲状腺激素血症(高TSH血症)组22例、正常甲状腺疾病综合征(低T3综合征)组10例),另选取同期性别、年龄与之相匹配的正常儿童50例设为对照组,观察各组儿童智能发育、身长发育情况,分析甲状腺功能减低对儿童生长发育的影响。结果:3月龄,高TSH血症组、CH组、低T3综合征组智力发育指数(MDI)、心理发育指数(PDI)积分均低于对照组,6月龄、1周岁高TSH血症组、CH组及低T3综合征组患儿MDI、PDI积分与对照组比较无统计学意义;3月龄,高TSH血症组、CH组、低T3综合征组身长、体重、头围均低于对照组,6月龄、1周岁高TSH血症组、CH组及低T3综合征组患儿身长、体重及头围与对照组比较无统计学意义。结论:经新生儿筛查为甲状腺功能减低的患儿,出生3个月内智能发育、身长、体重及头围均落后于正常健康儿童,但这种情况是暂时的,会在出生后3个月迅速发育,并在6个月~1周岁基本能与正常健康儿童生长发育水平持平。
Objective: To investigate the growth and development of children with hypothyroidism. Selection Methods: June 2014~June 2017 in affiliated hospital of Jining medical university diagnosed by neo-natal screening for thyroid dysfunction children 85 cases as group (high blood thyroid stimulating hormone (TSH levels) group 22 cases, 53 patients with primary hypothyroidism (CH) group and normal thyroid disease syndrome (low T3 syndrome) group of 10 cases of), another 50 normal chil-dren matched in gender and age were selected as the control group. The development of intelli-gence and body length of children in each group were observed, and the effect of hypothyroidism on the growth and development of children was analyzed. Results: At 3 months of age, the scores of in-tellectual development index (MDI) and psychological development index (PDI) in the hyperTSHe-mia group, CH group and low T syndrome group were lower than those in the control group. At 6 months, 1 year of age, the scores of MDI and PDI in the hyperTSH aemia group, CH group and low T syndrome group were not statistically significant compared with those in the control group. At 3 months of age, the body length, body weight and head circumference of the hyperTSH aemia group, CH group, low T group and syndrome group were lower than those of the control group. At 6 months of age, 1 year of age, the body length, body weight and head circumference of the hyperTSH aemia group, CH group and low Ts syndrome group were not statistically significant compared with the control group. Conclusion: Neonatal screening for children with thyroid function, intelligence de-velopment within three months of birth, height, weight and head circumference are lagging behind the normal healthy children, but this situation is temporary, will be rapid development in the three months after birth, and in the age of 6 months to 1 basic and normal healthy children’s growth and development level.
