1例子宫炎性肌纤维母细胞瘤病例介绍

Abstract:
炎性肌纤维母细胞瘤(inflammatory myofibroblastic tumor,IMT)是一种罕见的低恶性潜能的梭形细胞肿瘤,由分化的肌纤维母细胞梭形细胞和大量浆细胞和(或)淋巴细胞细胞组成。病因尚不清楚,可能诱因有特殊细菌或EB病毒感染、手术、损伤、炎症、异常修复、染色体畸变等。最常发生的部位是肺、肠系膜、大网膜和腹膜后。子宫炎性肌纤维母细胞瘤较少发生。免疫组织化学检测ALK表达和/或荧光原位杂交(FISH)检测ALK重排是炎性肌纤维母细胞瘤的诊断依据。本病首选手术治疗,通过手术完全切除病灶,大部分患者可治愈,但有复发、恶变、转移甚至致死可能。作为妇产科医生应该了解这种肿瘤并知道如何处理它。
Inflammatory myofibroblastic tumor (IMT) is a rare spindle cell neoplasm with low malignant po-tential, and it is composed of proliferative myofibroblasts and mixed inflammatory cell infiltrate. The cause is not clear, but it may be caused by special bacteria or EB virus infection, operation, in-jury, inflammation, abnormal repair, chromosome aberration. IMT may arise in multiple organs, most commonly lungs, mesentery, omentum, and retroperitoneum. Rarely IMT arise in the uterus. ALK expression by immuno-histochemistry and/or ALK rearrangement by fluorescence in situ hy-bridization (FISH) are diagnostic of IMT. The first choice for surgical treatment of this disease, through surgical complete removal of the focus, most of the patients can be cured, but there are re-currence, malignant transformation, metastasis and even death. As an obstetrician and gynecologist, you should understand this tumor and know how to deal with it.
