EVALUATION OF PSYCHIATRIC CONSULTATIONS OF MEDICAL INPATIENTS IN A UNIVERSITY HOSPITAL

Abstract:

Objective: This study aimed to investigate psychiatric consultations provided in a university hospital by age, sex of the patients, hospitalization reasons, hospitalization departments, reason of psychiatric consultation, consultation result diagnosis and treatment distributions. Method: The consultations requested from the Consultation liaison psychiatry department were reviewed retrospectively for the patients who were hospitalized in Ankara University School of Medicine (AüTF) hospitals between May 2, 2017 and October 31, 2017. Patients’ age, gender, reason for hospitalization, hospitalization departments, reason for referral consultation, consultation result diagnosis and treatment were obtained. The data were statistically evaluated with SPSS 16. Results: Of the 1101 cases, 51,4% (n=566) were female and 48,6% (n=535) were male. 28,6% of the consultations were requested from surgical clinics and 71,4% were from internal medicine departments, the most consultant departments were Chest Diseases (9,4%), Endocrinology and Metabolism Diseases (8,2%), Hematology (7,2%), General Surgery (6,6%) and Cardiology (6,2%). Reasons of psychiatric consultations were depressive mood (23,5%), anxiety (8,4%) and sleeping problems (7,8%) mostly. 34,7% (n=382) of the cases had had a psychiatric diagnosis before the psychiatric consultation. 22,5% of the cases didn’t get a psychiatric diagnosis after the psychiatric consultation. The most common diagnoses were depression (28,6%), anxiety disorders (14,4%), delirium (9,5%) and adjustment disorder (5,4%). 26,1% of the evaluated cases were not offered any medication, 38% (n=426) of the evaluated cases were offered antidepressant drugs and 16% (n=178) of them were offered antipsychotic drugs. It was thought that the gap between making a psychiatric diagnosis and offering any medication rates was due to the cases to which any medication couldn’t be offered because of various reasons. The most frequently prescribed drugs were escitalopram (13,7%), sertraline (13,3%), haloperidol (8,6%), lorazepam (4,9%) and quetiapine (4,2%). Conclusion: Departments that require the most consultation are Chest Diseases, Endocrinology and Hematology. Chronic diseases and malignancies were the most common reasons for hospitalization. High psychiatric diagnosis and treatment rates emphasized the importance of the holistic approach.
