This syndrome is a disorder in which a person holds a delusion that a friend, spouse, parent, or other close family member has been replaced by an identical-looking impostor. The delusion is common in patients diagnosed with various psychiatric disorder and the etiology is described as multifactorial. In this paper, we describe a Capgras Syndrome case, co-morbid with vitamin B12 deficiency which responded to pharmacological treatment with B12 replacement. In literature, there are few reports which call attention to the relation of Capgras syndrome and B12 deficiency. In the restricted knowledge of the etiology of this syndrome, we want to address the importance of medical evaluation of the patients with this syndrome
