摘要 目的 探讨绝经后妇女骨密度(BMD)与血液中血小板(PLT)计数、平均血小板体积(MPV)的相关性。 方法 对2017年8月-2018年6月我院417例青岛汉族绝经后妇女进行回顾性研究,根据双能X线BMD仪测定髋部(股骨颈、股骨干、股骨粗隆、全髋区和Wards三角区)及腰椎(L1~4)BMD的结果,按照WHO推荐的骨质疏松诊断标准将其分为骨质正常组(A组)、骨质减少组(B组)及骨质疏松组(C组),分别为160、149、108例,同时对所有患者血液中PLT计数、MPV等进行统计。对相关指标进行Pearson相关分析、偏相关分析和线性回归分析。 结果 三组患者年龄、BMI、血液中PLT计数、MPV、髋部(股骨颈、股骨干、股骨粗隆、全髋区和Wards三角区)及腰椎(L1~4)BMD比较差异有统计学意义(F=43.41~356.20,P<0.05)。Pearson相关分析表明,所有患者血液中PLT计数与髋部(股骨颈、股骨干、股骨粗隆、全髋区和Wards三角区)及腰椎(L1~4)BMD呈负相关(r=-0.248~-0.171,P<0.05)。患者血液中MPV与髋部(股骨颈、股骨干、股骨粗隆、全髋区和Wards三角区)、腰椎(L1~4)BMD呈正相关(r=0.174~0.298,P<0.01)。校正年龄和BMI后,偏相关系数分析显示,患者血液中PLT计数和髋部Wards三角区BMD呈负相关(r=-0.179,P<0.05);血液中MPV和腰椎(L1~4)BMD呈正相关(r=0.227,P<0.05)。线性多元逐步回归分析显示,患者血液中PLT计数与髋部(股骨颈、Wards三角区)、腰椎(L1~4)BMD呈负相关(B=-0.007~-0.001,P<0.05);患者血液中MPV与全髋区、股骨干、腰椎(L1~4)BMD呈正相关(B=0.016~0.026,P<0.05)。 结论 绝经后妇女BMD与血液中PLT计数、MPV可能存在相关性。 Abstract:Objective To investigate the correlation of bone mineral density (BMD) with platelet count (PLT) and mean platelet volume (MPV) in blood in postmenopausal women. Methods A retrospective analysis was performed for 417 postmenopausal Han women in Qingdao, China, who were treated in our hospital from August 2017 to June 2018. According to the results of BMD of the hip (femoral neck, femoral shaft, femoral trochanter, total hip, and Wards triangle) and the lumbar vertebrae (L1-4) measured by dual-energy X-ray absorptiometry, the women were divided to normal bone mass group (group A) with 160 women, osteopenia group (group B) with 149 women, and osteoporosis group (group C) with 108 women, and PLT and MPV in blood were measure for all patients. Pearson correlation analysis, partial correlation analysis, and linear regression analysis were performed for related indices. Results There were significant differences between the three groups in age, BMI, PLT, MPV, and BMD of the hip (femoral neck, femoral shaft, femoral trochanter, total hip, and Wards triangle) and the lumbar vertebrae (L1-4) (F=43.41-356.20,P<0.05). The Pearson correlation analysis showed that PLT in blood was negatively correlated with BMD of the hip (femoral neck, femoral shaft, femoral trochanter, total hip, and Wards triangle) and the lumbar vertebrae (L1-4) (r=-0.248--0.171,P<0.05), and MPV in blood was positively correlated with BMD of the hip (femoral neck, femoral shaft, femoral trochanter, total hip, and Wards triangle) and the lumbar vertebrae (L1-4) (r=0.174-0.298,P<0.01).
