Abstract:

摘要 目的 探究幽门螺旋杆菌(HP)感染与帕金森病(PD)患者运动症状严重程度的相关性。 方法 选取2016年1月—2019年2月日照市人民医院神经内科收治的初次发病且尚未用药治疗的PD患者80例(PD组)。同时选取同期在日照市人民医院进行健康查体者80例作为对照组。收集受检者的临床资料,进行13C呼气试验检测、HP抗体、维生素B12水平,记录所有患者的帕金森病评定量表UPDRS Part Ⅲ评分,并分析HP感染与UPDRS Part Ⅲ评分、维生素B12水平等的相关性。 结果 胶体金法检测结果显示,PD组与对照组HP阳性率分别为66.3%、37.5%,PD组患者HP感染率明显高于对照组(χ2=4.234,P<0.05);13C呼气试验检测结果显示,PD组与对照组HP阳性率分别为63.8%、36.3%,PD组患者HP感染率明显高于对照组(χ2=4.068,P<0.05);PD患者HP感染组的UPDRS Part Ⅲ评分与非感染组相比明显上升(P<0.05);PD组血清中维生素B12水平与对照组比较,差异无显著性(P>0.05);PD组中HP阳性组患者的血清维生素B12水平与阴性组比较,差异无显著性(P>0.05)。 结论 HP感染与帕金森病患者的运动症状严重程度相关,HP感染可加重PD患者的运动并发症程度,HP感染参与PD发病可能与维生素B12水平无关。
Abstract:Objective To explore the relationship between Helicobacter pylori (HP) infection and the severity of motor symptoms in patients with Parkinson’s disease (PD). Methods This study included 80 patients with PD (PD group) who had first onset and had not been treated with drugs and who were admitted to Department of Neurology, Rizhao People’s Hospital, from January 2016 to February 2019. At the same time,80 people who underwent physical examination in Rizhao People’s Hospital during the same period were selected as control group. Clinical data in both groups were collected, and 13C breath test and measurements of HP antibody and vitamin B12 were also performed. Unified Parkinson’s Disease Rating Scale (UPDRS) Part Ⅲ scores of all patients were recorded. The relationship of HP infection with UPDRS Part Ⅲ score and vitamin B12 level was analyzed. Results The results of colloidal gold method showed that the positive rates of HP in the PD group and the control group were 66.3% and 37.5%, respectively. The infection rate of HP in the PD group was significantly higher than that in the control group (χ2=4.234,P<0.05). The results of 13C breath test showed that the positive rates of HP in the PD group and the control group were 63.8% and 36.3%, respectively. Likewise, the PD group had a significantly higher infection rate of HP compared with the control group (χ2=4.068,P<0.05). In the PD group, HP-infected patients had a significantly higher UPDRS Part Ⅲ score than non-infected patients (P<0.05). There was no significant difference in the serum level of vitamin B12 between these two groups (P>0.05). In addition, the level of vitamin B12 was not significantly different between HP-positive and HP-negative patients in the PD group (P>0.05). Conclusion HP infection is associated with the severity of motor symptoms in patients with PD, which aggravates the degree of motor complications. However, involvement of HP infection in the pathogenesis of PD may not be related to the level of
