膝内侧半月板与软骨退变损伤的相关性研究

Abstract:

摘要 目的 观察膝内侧半月板退变损伤程度及径向撕裂形态、部位与内侧间室软骨退变损伤程度之间的关系,为膝骨关节炎的预防与治疗提供依据。 方法 2016年6月—2018年6月,连续观察于我院行膝关节镜下内侧半月板手术患者266例,共282膝,其中男122膝,女160膝,年龄41~80岁,年龄中位数57岁;所有患者均无明确的外伤史。结合术前MRI及术中镜下观察情况,按照内侧半月板退变损伤情况分为2组:变性组25膝,撕裂组257膝;根据ISAKOS半月板撕裂类型建议,按撕裂方向将撕裂组257膝分为2组:径向组171膝,非径向组86膝;按撕裂部位将径向组171膝分为3组:后根组50膝,后角组92膝和其他组(前角和体部)29膝。另外,按年龄分为2组:中年组175膝,老年组107膝。软骨损伤程度依据ICRS分级标准进行评价。将各组膝内侧半月板与软骨退变损伤的关系进行统计分析。 结果 内侧半月板退变程度、径向撕裂形态以及后部撕裂与软骨损伤程度有相关性(r=-0.307~0.195,P<0.05)。软骨损伤程度比较,撕裂组较变性组严重,径向组较非径向组严重,后根组较后角组严重,后角组较其他组严重,差异有统计学意义(Z=-2.808~-2.226,P<0.05)。软骨退变损伤程度与年龄具有相关性(r=0.233,P<0.05)。老年组软骨退变损伤程度较中年组严重,差异具有统计学意义(Z=-3.899,P<0.05)。 结论 临床上对于明确的半月板后角或根部径向撕裂患者,建议进行包括外科手术在内的早期治疗及相关措施干预,以有助于延缓骨关节炎的发生和进展。
Abstract:Objective To investigate the correlation of the degree of degenerative injury of the medial meniscus and the shape and location of radial tear with the degree of degenerative injury of cartilage in the medial compartment, and to provide a basis for the prevention and treatment of knee osteoarthritis. Methods A total of 266 patients (282 knees) who underwent knee arthroscopic surgery for the medial meniscus in our hospital from June 2016 to June 2018 were enrolled, with 122 knees in male patients and 160 knees in female patients. The patients were aged 41-80 years, with a median age of 57 years, and no patient had a history of trauma. According to the degree of degenerative injury of the medial meniscus on preoperative MRI and intraoperative arthroscopy, the patients were divided into degeneration group (25 knees) and tear group (257 knees); according to the ISAKOS Meniscus Classification System, the tear group was further divided into radial group (171 knees) and non-radial group (86 knees); according to the location of radial tear, the radial group was further divided into root of posterior horn group (50 knees), posterior horn group (92 knees), and other (anterior horn and body) group (29 knees). According to age, the patients were divided into middle-aged group (175 knees) and elderly group (107 knees). The degree of cartilage injury was evaluated according the ICRS system. A statistical analysis was performed for the correlation between degenerative injuries of the medial meniscus and the cartilage. Results The degree of the degenerative injury of the medial meniscus, shape of radial tear, and posterior tear were correlated with the degree of cartilage injury (r=-0.307--0.195,P<0.05).
