摘要 目的 探讨覆膜及静脉支架治疗罕见髂静脉压迫综合征(IVCS)深静脉血栓形成并动静脉瘘的效果。 方法 对1例IVCS深静脉血栓形成并动静脉瘘的病人采用股深动脉内植入覆膜支架封堵动静脉瘘口及开通闭塞的髂股静脉并植入静脉支架的方法进行治疗,观察治疗的效果。 结果 动脉内植入覆膜支架和使用静脉支架开通闭塞的髂股静脉能够缓解IVCS深静脉血栓形成并动静脉瘘病人下肢肿胀的症状,其中开通髂股静脉对于缓解病人症状作用更加明显。 结论 IVCS深静脉血栓形成并动静脉瘘的机制目前尚不清楚,相比于采取覆膜支架封堵动静脉瘘,使用静脉支架开通闭塞的髂股静脉对病人的临床意义更大。 Abstract:Objective To investigate the clinical effect of covered and venous stents in the treatment of deep vein thrombosis and arteriovenous fistula in patients with rare iliac vein compression syndrome (IVCS). Methods An IVCS patient with deep vein thrombosis and arteriovenous fistula was given covered stent implantation in the deep femoral artery to occlude the arteriovenous fistula orifice and venous stent implantation to open the occluded the iliofemoral vein. The treatment outcome was observed. Results Covered stent implantation in the deep femoral artery and venous stent implantation to open the occluded the iliofemoral vein effectively alleviated the symptom of swelling in the lower extremities in the IVCS patient with deep vein thrombosis and arteriovenous fistula, and opening of the occluded iliofemoral vein had a better effect in alleviating the patient’s symptoms. Conclusion The mechanism of deep vein thrombosis and arteriovenous fistula in patients with IVCS remains unclear. Compared with covered stent for the occlusion of arteriovenous fistula, venous stent for opening of the occluded iliofemoral vein has a higher clinical value in patients
