阳痛方外敷干预治疗急性痛风性关节炎的临床研究
Clinical Study on the Treatment of Acute Gouty Arthritis by External Application of Yangtong Fang

目的：观察阳痛方外敷干预治疗急性痛风性关节炎的临床疗效。方法：将69例急性痛风性关节炎患者随机分为治疗组和对照组。两组均以塞来昔布胶囊200 mg qd po为基础治疗，治疗组在基础治疗的同时，予以发作关节阳痛方外敷，对照组在基础治疗的同时，加用塞来昔布胶囊200 mg，每日一次。治疗7天后分析患者疼痛评分、中医证候积分评分和实验室检测指标(血尿酸、血沉、IL-6)变化，并评估临床疗效。结果：治疗24小时，两组疼痛程度均有明显改善(P < 0.05)，治疗组优于对照组(P < 0.05)，治疗7天后，治疗组和对照组在血尿酸、血沉、IL-6、中医证候积分以及疼痛评分方面无显著差异(P > 0.05)，同组治疗前后对比，均有显著差异(P < 0.05)，治疗组无明显不良反应，对照组有3例出现消化道症状。结论：阳痛方干预治疗急性痛风性关节炎见效更加迅速，可明显改善关节肿疼症状，安全性也更高。
Objective: To observe the clinical effect of external application of Yangtong fang in the treatment of acute gouty arthritis. Methods: 69 patients with acute gouty arthritis were randomly divided into treatment group and control group. Both groups were treated with celecoxib 200 mg qd Po as the basic treatment. The treatment group was treated with external application of Yangtong fang at the same time of basic treatment, while the control group was treated with celecoxib 200 mg once a day. After 7 days of treatment, the changes of pain score, TCM syndrome score and laboratory test indexes (UA, ESR, IL-6) were analyzed, and the clinical effect was evaluated. Results: After 24 hours of treatment, the degree of pain in the two groups was significantly improved (P < 0.05), and the treatment group was better than the control group (P < 0.05). After 7 days of treatment, there was no significant difference in UA, ESR, IL-6, TCM syndrome score and pain score between the treatment group and the control group (P > 0.05), and there was significant difference before and after treatment in the same group (P < 0.05). There was no significant adverse reaction in the treatment group, and there was no significant difference in the control group. Digestive tract symptoms were found in 3 cases. Conclusion: Yangtong fang intervention treatment of acute gouty arthritis is more effective, can significantly improve the symptoms of swelling and pain, the safety is also higher.
