Solid Organ Transplantation in Patients With a History of Lymphoma

Abstract:

INTRODUCTION Section: ChooseTop of pageAbstractINTRODUCTION < 700,000 solid organ transplantations have been performed in the United States since 1988.1 More than 33,000 transplantations were performed in 2016 alone. The indications for solid organ transplantation are also expanding, and transplantations are being performed in people for whom these procedures would have been contraindicated in the past. The presence of malignancy or a past history of malignancy may be a relative or absolute contraindication to solid organ transplantation. This is related to concerns about the possibility of an increased risk of cancer recurrence or de novo malignancy after transplantation in the setting of severe immunosuppression. In addition, there are concerns about a higher rate of transplantation-related morbidity and mortality from infections as well as toxicity related to prior chemotherapy and radiation. Finally, there are concerns that solid organ transplantation for high-risk recipients may result in poor institution results that are readily available to a variety of sources including private payers and the general public.2 Despite these concerns, successful transplantations in patients with a history of treated malignancy were described > 40 years ago.3 Although certain patients with a prior history of malignancy may be at higher risk for cancer recurrence and death, a pre-existing malignancy is not always an absolute contraindication for solid organ transplantation, and 1% to 6% of patients who undergo solid organ transplantation have a history of malignancy.4-10 Oncologists will usually be consulted when patients with a history of cancer are being evaluated for transplantation. This review will focus on solid organ transplantation in patients with a history of lymphoma.
