20例肠道Crohn病的MSCT诊断

Abstract:
摘要 目的:分析肠道Crohn病的多层螺旋CT(MSCT)影像表现,提高对其并发症的认识.方法:回顾性分析20例经内镜、手术病理证实的大、小肠Crohn病患者的MSCT资料,观察病变累及的部位、范围及强化方式,并对比不同年龄组肠周并发症特点.结果:在20例肠道Crohn病的病例中,15例发生在回肠中下段,其中1例累及盲肠,5例为结肠;MSCT所见包括肠壁增厚(20/20),肠壁异常强化(20/20),肠壁分层(16/20), 多型强化混合存在,假憩室(4/20),系膜血管增生及“梳样征”(12/20),肠系膜淋巴结增生(18/20).并发症表现为肠瘘(8/20),腹腔脓肿(5/20),其中1例合并脐尿管膀胱脓肿,肠梗阻(3/20),其中2例为小肠粘连性肠梗阻.60岁以上组6例,5例发生肠穿孔,60岁以下组14例,发生肠穿孔3例. 两组并发肠穿孔比较,有统计学差异(??2=6.706,P<0.05).结论:MSCT影像学表现能正确诊断肠道Crohn病,可与其他肠道疾病相鉴别,判断活动度、急时诊断并发症及评价疗效并为临床治疗提供依据.
