河南省居民住院卫生服务未利用情况及影响因素分析*
Unused inpatient service and its influencing factors of residents in Henan Province

目的:了解河南省居民应住院未住院的卫生服务未利用情况及影响因素。方法:采用多阶段分层、整群抽样的方法抽取河南省18个地区共33 426人,调查其基本情况及2013年应住院未住院原因、次数等卫生服务未利用情况并进行分析。结果:河南省居民2013年的应住院未住院率为19.57%。应住院未住院的主要原因为经济困难(50.47%)、没有必要(22.79%)。单因素分析结果显示年龄、是否患有慢性病、是否贫困、行动方面、从事日常活动和身体是否疼痛或不舒服有统计学意义(P<0.05)。多因素logistic回归分析显示影响居民应住院未住院情况的因素有年龄、身体疼痛或不舒服和是否贫困(P<0.05)。结论:是否贫困和身体疼痛或不舒服是影响河南省居民应住院未住院的重要因素,要加强对贫困人口的医疗救助力度及对居民自身健康状况的关注力度,提高卫生服务的利用率。
Aim: To understand the unused inpatient service and the influencing factors of the residents in Henan Province.Methods: A total of 33 426 residents in 18 regions from Henan Province were collected by multi-stage stratified cluster sampling.The basic conditions of the residents and the reasons for the unused inpatient service in 2013 were investigated.Results: For residents in Henan Province in 2013,the rate of unused inpatient service were 19.57%.Residents should be hospitalized but not mainly due to economic difficulties(50.47%),and there was no need for hospitalization(22.79%). Single factor analysis showed that age, whether suffering from chronic diseases, whether poor or not, whether pain or discomfort in daily activities were statistically significant(P<0.05).Multiple logistic regression analysis showed that the factors affected the hospitalization of residents were age, whether the body was pain or not, and whether poor or not(P<0.05).Conclusion: The degree of physical pain or discomfort and whether poor or not have become important factors that affect the hospitalization of residents in Henan Province.In order to improve the utilization of health services,we should strengthen the medical assistance for the people in poverty, and pay attention to the health of the residents
