Abstract:

目的:探讨脑电图(EEG)和弥散张量成像(DTI)在急性一氧化碳中毒迟发性脑病(DEACMP)中的诊断和预后评估价值。方法:选取2010年1月至2014年12月在新乡市第一人民医院住院的DEACMP患者60例,行甲强龙治疗,对其治疗前后分别行MMSE、Barthel功能评分和EEG、DTI成像。结果:治疗后患者MMSE、Barthel评分均有改善(P<0.05),EEG和DTI成像的FA值治疗后也明显改善(P<0.05)。治疗前后MMSE评分均与治疗前后ADC值有关(r=0.465、0.392、0.368、0.345,P<0.05),治疗前Barthel评分与治疗后ADC值有关(r=0.274,P<0.05)。治疗前患者的EEG结果与治疗前后DTI检测的ADC值有关联(P<0.05)。结论:EEG和DTI成像对DEACMP的病情判断、预后评估及临床治疗均具有重要意义。
Aim: To investigate the value of EEG and DTI in the diagnosis and prognosis of delayed encephalopathy after acute carbon monoxide poisoning(DEACMP).Methods: A total of 60 DEACMP inpatients were received from January 2010 to December 2014. MMSE, ADL function evaluation, EEG and DTI were performed before and after treatment with methylprednisolone.Results: The MMSE and ADL scores were obviously improved after treatment(P<0.05), and EEG, FA values of DTI imaging also improved significantly after treatment(P<0.05). MMSE scores before and after treatment were significantly correlated with ADC values(r=0.465,0.392,0.368,0.345,P<0.05). Barthel scores before treatment were significantly correlated with ADC values after treatment(r=0.274,P<0.05). The outcome of EEG before treatment was significantly associated with ADC values of DTI before and after treatment(P<0.05).Conclusion: Both EEG and DTI imaging are of significant value in condition judgment, prognosis and clinical treatment of DEACMP
