C3肾小球病患者血清MASP2、MASP3水平检测

Abstract:
目的:探讨甘露糖结合凝集素(MBL)途径相关蛋白甘露糖结合丝氨酸蛋白酶-2(MASP2)、MASP3在C3肾小球病(C3G)患者血清中的表达及其在C3G发病中的作用。方法:回顾性分析15例C3G患者(14例C3肾小球肾炎和1例致密物沉积病)的临床资料,用ELISA测定肾活检当天血清MASP2、MASP3水平,分析MASP2、MASP3与临床及病理指标的关系。10名健康志愿者为对照。结果:C3G患者血清MASP2、MASP3水平均高于对照组(P<0.05)。血清MASP2、MASP3水平与肾小球滤过率及血清补体C3水平均无关(P>0.05),血清MASP3水平与24 h尿蛋白定量呈正相关(r=0.563,P=0.036)。C3荧光强度高或有基底膜内电子致密物沉积的C3G患者血清MASP2水平较低(P<0.05)。结论:C3G患者尿蛋白量随着血清MASP3的升高而升高,提示MBL途径可能参与该病的发生发展; MASP2水平的异常降低可能与C3G病理损伤程度加重有关。
Aim:To investigate the serum mannan-binding lectin serine protease-2(MASP2),MASP3 of patients with C3 glomerulopathy(C3G)and their roles in the pathogenesis of C3G.Methods:The clinical and pathological data of 15 patients with C3G(14 cases of C3 glomerulonephritis and 1 case of dense deposit disease)were collected.Ten volunteers were chosen as healthy control.The levels of serum MASP2 and MASP3 of the 2 groups were detected by ELISA on the day of renal biopsy. The relationship between MASP2,MASP3 with the clinical and pathological parameters was evaluated.Results:Serum MASP2 and MASP3 in patients with C3G were generally higher than those in healthy controls(P<0.05). There was no significant correlation between serum MASP2,MASP3 with serum C3 or estimated glomerular filtration rate(P>0.05); serum MASP3 was positively correlated with 24-hour urinary protein(r=0.563,P=0.036). The serum MASP2 level was lower in the C3G patients with higher C3 deposition intensity or presence of basement membrane deposition(P<0.05).Conclusion:Mannan-binding lectin pathway may be involved in the pathogenesis of C3G.The abnormal decrease of MASP2 level may be related to the worsening of pathological damage of C3G
