超声弹性成像与磁共振弥散成像对宫颈癌的诊断价值
Value of ultrasound elastography with magnetic resonance DWI in diagnosis of cervical cancer

目的:探讨经阴道超声弹性成像(TVES)与磁共振弥散成像(DWI)在宫颈癌诊断中的应用价值。方法:选取病理证实的56例宫颈癌患者,30例拟行子宫肌瘤切除术、宫颈液基细胞学检查(TCT)良性的患者进行TVES和DWI检查,分析两种影像学方法对宫颈癌的诊断效能。结果:TVES诊断宫颈癌的敏感度、特异度、准确度分别为85.7%、83.3%、84.9%,DWI诊断宫颈癌的敏感度、特异度、准确度分别为92.8%、93.3%、93.0%,二者比较差异无统计学意义(P>0.05); TVES、DWI判断宫颈癌的ROC曲线下面积(95%CI)分别为0.915(0.852～0.978)、0.931(0.866～0.996),均有较高的诊断价值。结论:TVES及DWI有助于宫颈癌的诊断和鉴别诊断。
Aim: To assess the value of transvaginal ultrasound elastography(TVES)and magnetic resonance diffusion weighted imaging(DWI)in the diagnosis of cervical cancer.Methods: A total of 56 cases of cervical cancer, and 30 cases of uterine leiomyoma with negative result of thinprep cytology test(TCT)who were scheduled for hysteromyomectomy(the control)were underwent TVES and DWI examinations, the accuracy of the two methods in the diagnosis of cervical cancer was analyzed.Results: The sensitivity, specificity and accuracy of TVES were 85.7%, 83.3%, and 84.9%, respectively, those of DWI were 92.8%, 93.3%, and 93.0%, respectively,and there was no statistical difference between the two methods(P>0.05). The areas under ROC curve of TVES and DWI were 0.915(0.852-0.978)and 0.931(0.866-0.996), with high accuracy of diagnosis.Conclusion: TVES and DWI can be helpful for diagnosis and differential diagnosis of cervical cancer
