紧贴小转子截骨结合Wagnercone股骨假体治疗成人CroweⅣ型先天性髋关节发育不良

Abstract:

目的探讨采用紧贴小转子截骨结合Wagner cone股骨假体行人工全髋关节置换术(total hip arthroplasty,THA)治疗成人Crowe Ⅳ型先天性髋关节发育不良(developmental dysplasia of the hip,DDH)的近期疗效。 方法2009年11月-2012年9月,收治7例(9髋)成人CroweⅣ型DDH女性患者。年龄24~62岁,平均42岁。左髋2例、右髋3例,双髋2例。术前患髋Harris评分为(50.00±7.04)分。双下肢不等长,患侧较健侧短缩3~4 cm;平均3.4 cm,Trendelenburg征均呈阳性。X线片示患髋高位完全脱位。采用紧贴小转子截骨结合Wagner cone股骨假体行THA。 结果术后切口均Ⅰ期愈合,无下肢深静脉血栓形成、血管神经损伤、髋关节脱位、假体周围感染等并发症发生。7例患者均获随访,随访时间25~48个月,平均36.4个月。患者疼痛缓解,术前跛行步态明显改善,均恢复日常生活,双下肢基本等长。Trendelenburg征均为阴性。患髋Harris评分达(83.42±6.47)分,与术前比较差异有统计学意义(t=8.90,P=0.00)。X线片复查示,3例大转子滑移截骨处均于术后3~6个月愈合,随访期间假体骨长入良好,宿主骨与假体界面稳定牢固,无假体松动及下沉。 结论成人Crowe Ⅳ型DDH行THA时,当患肢较健肢短缩不大于4 cm时,通过充分松解挛缩软组织,紧贴小转子截骨,并结合Wagner cone股骨假体,能有效避免肢体长度纠正时对神经血管的损伤以及降低复位难度,近期疗效较满意,但假体远期生存率有待进一步观察
