刃厚头皮片和颞顶筋膜瓣在低发际小耳耳廓再造中的应用

Abstract:

目的探讨刃厚头皮片和颞顶筋膜瓣在低发际小耳耳廓再造术中的应用及疗效。 方法2010年7月-2015年4月,收治23例(23侧)低发际小耳畸形。男16例,女7例;年龄6~34岁,平均12岁。左侧10例,右侧13例。耳垂型小耳并低发际18例,耳甲腔型小耳并低发际4例,小耳甲腔型小耳并低发际1例。参照Nagata法二期耳廓再造术,一期行肋软骨耳廓支架成型和移植,6个月后行二期耳颅沟成型和头发区处理,在再造耳区发际线以上部分和颞枕部设计并切取与再造耳皮肤连续的刃厚头皮片,术中剪除再造耳头发区的真皮、毛囊和皮下组织层,再造耳前面去毛囊区和耳颅沟区行带颞浅血管的颞浅筋膜瓣覆盖,最后将刃厚头皮片覆盖于筋膜瓣表面缝合固定。 结果手术均顺利完成。两期手术切口均Ⅰ期愈合,无相关并发症发生。术后均获随访,随访时间6~20个月,平均12个月。再造耳术后无头发生长,形状均满意。 结论对于低发际小耳畸形,采用Nagata法二期耳廓再造时,二期手术中再造耳头发区形成刃厚头皮片和剪除真皮、毛囊及皮下组织层后,以颞顶筋膜瓣覆盖和刃厚头皮片移植可获得满意外形且无头发生长
