关节镜下前交叉韧带保留残端重建的二次关节镜检查研究

Abstract:

目的通过二次关节镜检查前交叉韧带(anterior cruciate ligament,ACL)保留残端重建术后重建韧带的完整性、张力、滑膜覆盖及色泽,探讨保留残端的必要性。 方法以2011年1月-2013年12月122例ACL重建术后1年行二次关节镜取内固定术者作为研究对象,其中61例重建术中保留了韧带残端(保留组),61例未保留残端(未保留组)。两组患者性别、年龄、侧别、体质量指数、损伤类型、受伤至重建术时间、KT-2000膝关节稳定性测量器检查胫骨前移距离等一般资料比较,差异均无统计学意义(P>0.05),有可比性。二次关节镜术中观察重建韧带情况,并参照敖英芳提出的再次关节镜检查评价标准进行评价。 结果根据敖英芳制定的再次关节镜检查评价标准,保留组获优34例,良22例,中4例,差1例;未保留组获优29例,良20例,中10例,差2例;两组比较差异无统计学意义(Z=—1.320,P=0.187)。 结论与未保留残端重建相比,保留残端重建的韧带在完整性、张力、滑膜覆盖、色泽方面无显著提高
