精子DNA完整性与精液参数及辅助生殖技术临床结局的关系 Association of Sperm DNA Integrity with Sperm Parameters and Clinical Outcomes of Assisted Reproductive Technique

Abstract:

目的:探讨精子DNA完整性与精液参数及体外受精-胚胎移植(IVF-ET)/卵胞浆内单精子注射(ICSI)临床结局的关系。方法:采用精子染色质结构分析(SCSA)检测148个辅助生殖技术(ART)周期中精子DNA的完整性,以精子DNA碎片指数(DFI)为参数,将患者分为分为高DFI组(DFI≥30%)和低DFI组(DFI<30%),根据采用不同的体外受精方式将两组又分为IVF亚组和ICSI亚组,比较组间精液参数、受精率、卵裂率、优胚率、种植率和临床妊娠率。结果:精液密度、精子活动力(a+b)与精子DFI呈负相关(P<0.01),精子畸形率与精子DFI呈正相关(P<0.01)。低DFI组的IVF亚组与高DFI组的IVF亚组相比,受精率(P<0.05)及优胚率(P<0.05)有统计学意义,其余各亚组间受精率、卵裂率及优胚率均无显著差异。各亚组间临床妊娠率及种植率差异均无统计学意义,但DFI≥30%的ICSI亚组临床妊娠率及种植率高于其它亚组。结论:精子DNA完整性与精液密度、活动力、形态呈负相关,对受精率及优胚率有明显影响,与妊娠结局无相关性,但建议DFI≥30%时,优先选择ICSI。在临床应用中,建议将DNA完整性分析作为精液分析的常用项目
