新生儿窒息后血清脑钠肽和N末端脑钠肽前体水平与窒息程度的关系 Correlation Between Asphyxia Degree and the Level of Serum BNP and NT-proBNP After Asphyxia

Abstract:

目的:探讨新生儿窒息后血清脑钠肽(BNP)和N-末端脑钠肽前体(NT-proBNP)水平与窒息程度的关系。方法:按就诊顺序编号,每间隔一例选取武汉大学中南医院新生儿科2014年12月至2016年12月住院患儿,共选取106例窒息患儿为观察组,其中轻度窒息46例、重度窒息60例,同时选取同期收治的无窒息史及心血管系统疾病的63例新生儿为对照组,除外水电解质紊乱和肝肾功能障碍,两组患儿均要求入院日龄<3d。对所有入选患儿在入院2h内及入院2周测定血浆BNP、NT-proBNP等并进行统计分析。结果:入院2h内,重度窒息组和轻度窒息组患儿血浆NT-proBNP和BNP水平均高于对照组,重度窒息组高于轻度窒息组,差异有统计学意义(P<0.05);入院第2周时,各组NT-proBNP和BNP均明显下降,但重度窒息组BNP仍高于轻度窒息组和对照组,差异有统计学意义(P<0.05),轻度窒息组与对照组差异无统计学意义(P>0.05)。NT-proBNP在三组间无统计学差异。结论:血浆BNP及NT-proBNP水平均能反映窒息患儿窒息严重程度,窒息越重,标志物水平越高,且NTproBNP有更高的特异性和敏感性,可作为具有重要的临床参考价值
