丙泊酚中/长链脂肪乳注射液用于剖宫产全麻诱导对产妇及新生儿的影响 Effects of Propofol Medium and Long Chain Fat Emulsion Injection on Maternal and Neonatal in the Induction of General Anesthesia for Cesarean Delivery

Abstract:

目的:探讨丙泊酚中/长链脂肪乳注射液用于剖宫产全麻的应用价值及其安全性,为临床合理用药提供参考。方法:丙泊酚中/长链脂肪乳注射液用于剖宫产全麻诱导,对比观察全身麻醉和硬膜外麻醉对产妇血液动力学及新生儿脐血血气和Apgar评分的影响,选择ASAⅠ-Ⅱ级足月妊娠初产妇60例,随机分为全身麻醉组(M组)和硬膜外麻醉组(E组),每组各30例。分别记录产妇入室后(T0)、麻醉诱导(T1)、手术切皮时(T2)、胎儿剖出即刻(T3)等时点的脉搏血氧饱和度(SpO2)、平均动脉压(MAP)、心率(HR)的变化。两组产妇均在新生儿娩出时抽取脐动、静脉血,进行血气分析,分别记录两组切皮至新生儿娩出时间(S-DI)、子宫切开至新生儿娩出时间(U-DI)、新生儿出生1min和5min Apgar评分。结果:全麻组和硬膜外组的S-DI时间分别为(5.4±1.5)min和(10.1±2.2)min,两组相比差异有显著意义(P<0.05),U-DI时间两组相比差异无统计学意义(P>0.05)。全麻组和硬膜外组新生儿脐动、静脉血血气分析和新生儿出生1 min和5 min Apgar评分两组比较,差异均无统计学意义(P>0.05)。结论:丙泊酚中/长链脂肪乳注射液2.0mg/kg切皮前单次静脉推注应用于剖宫产全麻诱导是安全有效的,母体血液动力学控制理想,对新生儿脐血血气和Apgar评分无明显影响
