ICU颅脑损伤患者应用不同肠内营养及益生菌制剂的临床效果 Outcomes of Early Different Types of Nutritional Support Applicated in Patients of ICU with Craniocerebral Injury

Abstract:

目的:探讨在重症监护室(ICU)中不同肠内营养(EN)成分对颅脑损伤患者的临床效应及不良反应等。方法:选择颅脑损伤患者共138例,随机分为瑞先(A)组、瑞先+益力多(B)组与安素(C)组,每组各46例,三组均通过鼻饲管连续饲肠内营养液作营养支持维持2-3周。观察治疗效果并作统计学分析,分别记录三组患者入室前、后血糖、胰岛素用量、白细胞计数(WBC)、降钙素原(PCT)、血红蛋白(Hb)、血清总蛋白(TP)、血清白蛋白(ALB)、肝肾功能、电解质、血脂、肱三头肌皮褶厚度(TSF)、体质量指数(BMI)、胸部X片变化等。观察胃肠道耐受情况、不良反应等。结果:营养治疗第7、15天C组ALB均明显低于B组对应值,差异有统计学意义(P<0.05);治疗第15天,C组BMI、TSF、TP、Hb明显低于A组、B组,WBC、PCT高于A、B组(P<0.05);B组TP、ALB、Hb明显高于A组,差异差异有统计学意义(P<0.05);C组患者在ICU住院时间明显长于A、B组,不良反应发生率也明显高于其他两组,差异有显著性(P<0.05)。结论:颅脑损伤患者早期应用瑞先加益力多行肠内营养可改善营养状况,减轻炎症反应,减少感染等并发症的发生及缩短在ICU的住院时间
