超声内镜在消化道病变诊断中的应用价值

目的 探讨超声内镜(endoscopic ultrasonography, EUS)对消化壁病变的诊断及指导治疗方式选择的临床价值。方法 对同济大学附属同济医院EUS检查发现的215例消化道黏膜及黏膜下病变进行超声内镜检查,通过与病理结果比较分析EUS的诊断价值,并探讨EUS对治疗方式选择的指导意义。结果 在食管、胃、十二指肠、直肠病变中,超声诊断结果与病理检查结果对比诊断准确率分别为94.5%(52/55)、95.7 %(66/69)、100.0 %(10/10)、98.6%(69/70),明显高于普通内镜及CT的诊断准确率。结论 超声内镜对消化道病变的来源及性质判断有较高准确性,且为隆起病变在内镜下选择何种治疗方案提供重要依据。
Objective To evaluate the application of endoscopic ultrasonography (EUS) in diagnosis and treatment of gastrointestinal diseases. Methods A total of 215 patients with gastrointestinal lesions detected by routine endoscopy undergoing EUS examination were included in this retrospective study. The diagnostic value of EUS was assessed by comparing EUS findings with pathological results. Results Compared with pathological examination, the accuracy of EUS examination for esophagus, stomach, duodenum and rectal lesions were 94.5%(52/55), 95.7% (66/69), 100.0%(10/10) and 98.6% (69/70) respectively, which was superior to that of rountine endoscopy of CT scan. The exact layer of origin and invasion of mural lesions shown by EUS were beneficial for GI physicians to choose appropriate therapeutic modalities. ConclusionEUS is capable of showing the origin and property of gastrointestinal lesions,so it is of value for effective and safe management of gastrointestinal diseases
