重症牙周炎牙周与种植治疗的矛盾和统一
Periodontal therapy and implant restoration of severeperiodontitis: conflict or accordance

重症牙周炎（SPe）是牙齿缺失的主要原因，对SPe患牙实行牙周保存治疗和拔除后种植修复一直是争论的焦点，牙周病医师技术不断提高使“无望”患牙得以保留，而种植医师希望早期拔牙以保留残余的牙槽骨利于种植。其实二者是统一的，都是治疗SPe恢复患牙功能的有效手段。本文从SPe“无望牙”牙周保存疗效，种植成功与种植体周围炎矛盾，种植前后牙周治疗对种植体“寿命”影响等方面论述SPe牙周与种植治疗的矛盾和统一，为SPe成功的牙周治疗和种植修复提供依据。
Severe periodontitis is a major cause of tooth loss. It has been the focus of debate to choose toothpreservation through periodontal therapy or implant restoration post extraction. Therapeutic technology of periodontistscontinues to improve for preserving the “hopeless” tooth, whereas the implantation specialists wish to extract the affectedtooth as early as possible to keep the limited alveolar bone for implantation. The two, in fact, are in accordance as theeffective means of treating severe periodontitis and recovering tooth function. This paper discussed the conflict andaccordance between periodontal therapy and implant restoration of severe periodontitis by reviewing the curative effectsof periodontal therapy of the “hopeless” tooth, the contradiction between implantation success and peri-implantitis, and theinfluence of periodontal systematic therapy pre- and post-implantation on implant “life-span”.
