采用微血管减压治疗椎动脉夹层动脉瘤致面肌痉挛一例

一例61岁女性患者，左侧面部发作性抽搐4年，头颅磁共振和脑血管造影提示左侧椎动脉夹层动脉瘤形成，压迫同侧面听神经变形移位，行微血管减压术，推开夹层动脉瘤并将其远端载瘤动脉粘附于岩骨硬脑膜，面神经充分减压。术后患者面肌痉挛完全缓解，随访6个月无症状复发。
Abstract: A 61-year-old female presented with 4 years history of left-sided hemifacial spasm. Head MRI and angiography indicated left vertebral artery dissecting aneurysm which compressed ipsilateral cranial nerves Ⅶ and Ⅷ. Microvascular decompression was performed. The dissecting aneurysm was pushed apart and the distal part of the parent artery was adhered to the dura on the petrosum. The compressed nerves were totally decompressed. The symptom of facial spasm was completely resolved immediately after surgery and did not recur during 6 months of follow up.
