爆炸伤创面愈合过程中mmp-9和tgf-β所起的作用和重量的变化

Abstract:
目的通过研究湿热环境基质金属蛋白酶-9(mmp-9)及转化生长因子-β(tgf-β)在创面愈合过程中的表达变化,探讨它们对创面愈合所起的作用以及两者之间的相互关系,为临床预防和治疗湿热环境下战伤创面提供理论依据。方法建立湿热环境爆炸伤创面的动物模型,收集伤后4、24、48h和5、7、14、21、28d创面渗液。采用明胶酶谱法和elisa法,分别对创面mmp-9和tgf-β进行检测。结果创面愈合过程中mmp-9和tgf-β有规律性变化,以伤后48h为高峰,tgf-β伤后7d出现第二高峰,伤后2周两者水平下降,并且出现分离现象。创面给予金属蛋白酶抑制剂(timp)的干预,在早期无明显作用,但在后期,即伤后2周,可以降低mmp-9、上调tgf-β的水平。结论创面愈合过程中,早期mmp-9和tgf-β水平上升促进了创面细胞的迁移,有利于创面炎性坏死组织的清除,可能是创面愈合的机制之一,而mmp-9水平的过度升高,不利于创面的愈合。合理外用timp对促进延迟愈合创面的愈合会取得良好的效果。
