Abstract:
目的建立原位杂交法检测胸液细胞中人端粒酶逆转录酶(htert)mrna的实验方法,初步评价该法识别胸液中肿瘤细胞的效果。方法取23例胸腔积液病例标本,以地高辛标记的寡核苷酸探针对胸液细胞htertmrna行两步原位杂交,再通过抗原-抗体-酶-显色途径,测出htertmrna信号。实验研究设严格的阴性、阳性对照,且与临床诊疗各自独立进行。结果本法对9例恶性胸腔积液病例检测的结果均为阳性,即无论细胞形态是否异常,镜下均可见到有细胞质深染及部分细胞核少许点状着色的细胞。此9例中,7例经胸液细胞形态学检查亦示阳性者,可同时观察到形态明显异常的肿瘤细胞,其余2例经反复胸液细胞形态学检查阴性,但胸膜活检确诊为恶性胸腔积液者,虽未见到形态明显异常细胞,但镜下可发现胞质深染、部分细胞核少许着色的细胞。14例良性胸腔积液病例的检测结果均为阴性。结论原位杂交法检测胸液细胞htertmrna可望作为一种临床细胞病理学的新方法,用以鉴别胸腔积液的良、恶性,同时判定肿瘤细胞的类型。
